The human gut hosts trillions of microorganisms that influence digestion, immune function, vitamin synthesis, and intestinal barrier integrity. When this balance is disturbed (by antibiotic use, illness, dietary changes, or stress), the result can include bloating, irregular bowel habits, and recurrent gastrointestinal discomfort.

Lactobacillus acidophilus is a lactic acid-producing bacterium naturally found in the human gut and oral cavity and is used in fermented foods. It produces lactic acid and antimicrobial peptides that help inhibit less desirable bacteria.

Bifidobacterium longum is one of the predominant bacterial species in the healthy human gut. It produces short-chain fatty acids that nourish intestinal cells, support intestinal barrier function, and interact with the gut immune system.

Streptococcus thermophilus is a lactic acid bacterium widely used for the fermentation of yoghurt. It produces lactase (an enzyme that breaks down lactose).

Lactitol is a non-absorbable sugar alcohol that reaches the colon largely unchanged. Once it reaches the colon, gut bacteria ferment it into short-chain fatty acids, which lower the pH of the colon and draw water into the bowel, producing a mild osmotic laxative effect. In hepatic encephalopathy, this process traps ammonia in the colon as ammonium, which is then excreted, thereby reducing systemic ammonia levels.

Take Awaytox Capsule 10s orally as directed. Lactitol is often started at a lower dose and titrated up to achieve 2–3 soft stools per day, usually in hepatic encephalopathy, where dose adjustment is critical. Take with a meal or as advised. When taken alongside antibiotics, separate the probiotic dose from the antibiotic by at least 2 hours. Maintain adequate fluid intake. Avoid very hot beverages around the time of dosing, as heat can reduce probiotic viability. Store as directed on the label, as some products require refrigeration.

The probiotic strains work through several complementary mechanisms. They produce lactic acid and short-chain fatty acids that lower local colonic pH, inhibiting the growth of less desirable bacteria. They compete with potential pathogens for attachment sites on the intestinal lining and for available nutrients. They produce antimicrobial peptides that directly inhibit certain bacteria. They also interact with the gut immune system and support the integrity of the intestinal barrier.

Lactitol passes through the upper gut unabsorbed. In the colon, gut bacteria ferment it into short-chain fatty acids, particularly acetate, propionate, and butyrate. These acidify the colon, drawing water into the bowel by osmosis (softening stool and easing constipation), and providing prebiotic substrate for beneficial bacteria. The acidic environment converts ammonia (NH3) (which can cross the gut wall) into ammonium (NH4+), which cannot, trapping it for excretion.

Research reveals that maintaining the right stool frequency is more important than achieving rapid bowel emptying when using osmotic laxatives in hepatic encephalopathy, with dose titration guided by 2–3 soft stools per day.

• High-fibre diet (BENEFICIAL): Supports overall gut health. Dietary fibre acts as a substrate for beneficial gut bacteria, complementing the prebiotic effect of lactitol.

• Very hot beverages (CAUTION): Heat can reduce probiotic viability. Avoid mixing the supplement with tea, coffee, or any drink above body temperature, as heat damages the live bacteria.

• Excess alcohol (CAUTION): Worsens dysbiosis and is harmful in liver disease. Heavy alcohol intake disrupts the gut microbiota balance that the supplement is meant to support and undermines its benefit in hepatic encephalopathy.

• Antibiotics (CAUTION): May reduce probiotic viability; separate by at least 2 hours. Taking both together can kill a substantial proportion of the live bacteria before they reach the colon.

• Other laxatives (CAUTION): Risk of excessive diarrhoea and electrolyte imbalance. Adding another laxative on top of lactitol can lead to dehydration, particularly in older adults.

• Immunosuppressants (CAUTION): Use with medical advice in severely immunocompromised patients. Reduced immune defences raise the theoretical risk of probiotic strains causing systemic infection.

• Severe immunocompromise (CAUTION): Theoretical risk of probiotic-related bacteraemia. This includes patients on chemotherapy, organ transplant recipients, and those with advanced HIV. Use only under specialist guidance.

• Central venous catheter in situ (CAUTION): Theoretical risk of catheter-related infection. The catheter provides a route for bacterial translocation into the bloodstream, even when the gut barrier is intact.

• Severe acute pancreatitis (CAUTION): Probiotic use has been associated with safety concerns in some settings. Avoid initiating probiotic therapy in the acute phase without specialist input.

• Galactosaemia (CONTRAINDICATED): Lactitol-containing products may not be suitable. The condition prevents safe metabolism of galactose, and certain lactitol formulations may pose a risk.

• Intestinal obstruction (CONTRAINDICATED): Osmotic laxatives can worsen obstruction. Increasing the volume of intestinal contents above a blockage raises the risk of bowel perforation.

Overdose of Awaytox Capsule 10s may cause severe diarrhoea, abdominal cramping, and dehydration. People may also experience electrolyte disturbance, especially if Awaytox Capsule 10s is taken for a prolonged period or in excess amounts. Consult your doctor if symptoms are severe or persistent, especially in older patients or those with kidney disease.

If you forget a dose, take it as soon as you remember. If it is close to the next dose, skip the missed dose and continue. Do not double the dose.
