Dapagliflozin + Metformin + Sitagliptin is a triple-combination antidiabetic medicine used to improve blood glucose control in adults with type 2 diabetes. It works by enhancing insulin sensitivity, reducing glucose production, improving incretin hormone action, and promoting glucose excretion through urine. This combination is typically prescribed when single or dual therapy is inadequate. It helps reduce HbA1c levels, supports weight control, and may lower cardiovascular risks when combined with dietary and lifestyle measures.
Dapagliflozin + Metformin + Sitagliptin is a powerful oral antidiabetic formulation designed to provide comprehensive glucose control in adults with type 2 diabetes mellitus. It combines three complementary classes of medicines, SGLT2 inhibitor (Dapagliflozin), Biguanide (Metformin), and DPP-4 inhibitor (Sitagliptin), to target multiple pathways involved in elevated blood sugar levels. This makes it particularly beneficial for individuals who have not achieved satisfactory glycaemic control with metformin alone or dual therapies.
Metformin reduces excess glucose production by the liver and improves insulin sensitivity, helping cells use glucose more effectively. Sitagliptin works by increasing incretin hormone levels, thus enhancing insulin release after meals and reducing glucagon secretion. Dapagliflozin promotes the removal of glucose through urine by blocking the SGLT2 transporter in the kidneys, supporting modest weight loss and lowering fasting blood sugar.
Regular use helps reduce HbA1c, fasting glucose, and post-meal spikes, providing round-the-clock control. The combination also offers additional metabolic support, including lowered blood pressure and reduced cardiovascular risk in suitable patients. It should always be taken under medical supervision, especially in people with kidney disease, dehydration risk, or severe gastrointestinal conditions. When combined with healthy eating habits, regular exercise, and weight monitoring, this triple therapy becomes an effective long-term solution for managing type 2 diabetes and preventing complications such as neuropathy, nephropathy, and cardiovascular disease.
This medicine is generally safe with most foods. However, alcohol significantly increases the risk of lactic acidosis when taken with metformin and should be avoided. Heavy carbohydrate meals may cause post-meal spikes and reduce the drug’s effectiveness. Grapefruit does not interact with this medicine, unlike some other antidiabetics.
Avoid or limit:
Some medicines may alter the effectiveness of this combination or increase the risk of side effects. Diuretics can worsen dehydration when combined with dapagliflozin. Insulin or sulfonylureas may cause hypoglycaemia when taken together. Certain blood pressure medicines may amplify dizziness or low-BP effects.
Inform your doctor if you are taking:
This combination should be used cautiously in patients with kidney disease, heart failure, dehydration, or recurrent urinary infections. Metformin increases the risk of lactic acidosis in severe kidney or liver impairment. Dapagliflozin may worsen dehydration or blood pressure issues. Sitagliptin may need dose adjustments in renal problems.
Use with caution in:
Your doctor will determine the appropriate daily dose based on your blood sugar levels, kidney function, and previous treatment history. The combination is usually taken once daily with meals to minimise stomach upset and ensure optimum glucose control. Do not change the dose without medical supervision.
Taking more than the prescribed dose of Dapagliflozin + Metformin + Sitagliptin can lead to serious side effects, including severe low blood sugar, dehydration, lactic acidosis (a rare but dangerous complication of metformin), or electrolyte imbalance. Seek immediate medical attention or visit the nearest emergency department. Do not attempt to self-treat at home, as timely supportive care and monitoring are essential for safety.
If you forget a dose, take it as soon as you remember. If it is close to the time of your next scheduled dose, skip the missed dose and resume your regular dosing schedule. Do not take two doses together to make up for the missed dose, as this may lead to low blood sugar or side effects. Monitor your glucose levels if you miss a dose and inform your doctor if frequent forgetfulness occurs.
