Eusave 400 Tablet

Eusave 400 Tablet is a phosphate-binding medication containing of Sevelamer hydrochloride or carbonate. It is primarily used to control serum phosphorus levels in patients with chronic kidney disease (CKD) undergoing dialysis. By binding to phosphorus in the digestive tract, it prevents the mineral from being absorbed into the bloodstream. This helps maintain a healthy balance of minerals in the body and prevents long-term complications associated with high phosphorus, such as bone disease and cardiovascular calcification.

Eusave 400 Tablet is a non-absorbed, calcium-free, and metal-free polymeric phosphate binder composed of 400mg of Sevelamer. It is an essential therapy for managing mineral and bone disorders in patients whose kidneys can no longer effectively filter excess waste.

Composition and Mechanism: Each tablet contains of Sevelamer. The medication consists of a non-absorbed cross-linked polymer. It works through ion exchange and hydrogen bonding to bind phosphate molecules within the gastrointestinal tract. Because the polymer itself is not absorbed into the blood, it carries the trapped phosphorus through the digestive system to be excreted in the faeces. Unlike older binders, it does not contain calcium or aluminium, reducing the risk of "calcium overload" or metal toxicity.

Major Therapeutic Uses: Eusave 400 Tablet is indicated for the control of hyperphosphatemia (high blood phosphorus) in adult patients with chronic kidney disease on hemodialysis or peritoneal dialysis. It is also used in some clinical settings to lower LDL (bad) cholesterol, as the polymer also binds to bile acids in the gut.

Benefits and Precautions: The primary benefit of Sevelamer is its ability to lower phosphorus without increasing serum calcium levels. This is critical for preventing the hardening of heart valves and arteries (calcification). However, a major precaution involves patients with bowel obstructions or severe gastrointestinal motility disorders. Because the medication expands slightly in the gut, it can worsen existing blockages or severe constipation.

Medical Supervision and Risks: This medication requires strict medical oversight, including monthly blood tests to monitor phosphorus, calcium, and bicarbonate levels. Significant risks include gastrointestinal complications such as perforation or impaction in rare cases. Patients must also be monitored for potential fat-soluble vitamin deficiencies (Vitamins A, D, E, and K), as the binder may slightly interfere with their absorption.

Common Side Effects (Mostly gastrointestinal):

• Nausea and Vomiting: Often occur during the first few weeks.

• Bloating: A feeling of fullness or gas in the abdomen.

• Constipation: Difficulty passing stools (very common).

• Diarrhoea: Frequent loose stools.

• Stomach Pain: General abdominal discomfort.

Uncommon Side Effects:

• Dyspepsia: Heartburn or acid reflux.

• Flatulence: Excessive intestinal gas.

• Fatigue: General feeling of tiredness.

Serious Side Effects (Requires immediate medical attention):

• Bowel Obstruction: Severe constipation, inability to pass gas, and intense abdominal cramping.

• Gastrointestinal Perforation: Severe, sudden abdominal pain, fever, and nausea.

• Allergic Reaction: Rash, hives, or swelling of the face and throat.

• Severe Dysphagia: Difficulty swallowing or the feeling of the tablet being stuck in the throat.

• Timing: MUST be taken with meals. It only works if it is in the stomach at the same time as food.
• Method: Swallow the tablet whole. Do not crush, chew, or break the tablet, as the polymer can expand and cause a choking hazard.
• Dose Division: If you eat three meals a day, your total daily dose will be divided across those three meals.
• Consistency: Take it with every meal and snack that contains phosphorus (nearly all protein-rich foods).
• Water Intake: Follow your dialysiscentre’ss fluid restriction guidelines, but ensure you have enough water to swallow the tablet comfortably.

• The "Meal Rule": Carry a small pill container with you so you never have to eat a meal without your binder.

• Manage Constipation: If you become constipated, talk to your dietitian or doctor immediately. They may suggest a renal-safe stool softener.

• Tablet Size: 400mg tablets can be large. If you have trouble swallowing, talk to your doctor; never crush them.

• Vitamin Timing: Take any daily multivitamins at least 1 hour before or 3 hours after Sevelamer to ensure they are absorbed properly.

Store Eusave 400 Tablet at room temperature (25°C). Keep the bottle tightly closed to protect the tablets from moisture, as the polymer can absorb humidity from the air. Keep out of reach of children.

• High-Phosphorus Foods: While Sevelamer binds phosphorus, eating excessive amounts of dairy, nuts, or sodas will overwhelm the medication’s capacity.

• Vitamin Absorption: May reduce the absorption of fat-soluble vitamins (A, D, E, K) and folic acid.

Ciprofloxacin

SEVERE

Sevelamer can reduce the absorption of this antibiotic by up to 50%. Take 2 hours apart.

Levothyroxine

MODERATE

May lower thyroid medicine levels. Take at least 4 hours apart from Sevelamer.

Mycophenolate

MODERATE

May reduce the effectiveness of this transplant medication.

Cyclosporine

CAUTION

Monitor blood levels of this immunosuppressant closely when starting Sevelamer.

• Bowel Obstruction: Strictly contraindicated for patients with an active intestinal blockage.

• Swallowing Disorders: Use with caution in patients with dysphagia or severe gastrointestinal motility issues.

• Major GI Surgery: Use with caution if you have recently had surgery on your stomach or intestines.

The dose is adjusted based on your blood phosphorus levels.

• Starting Dose: Typically 800mg to 1600mg per meal (two to four 400mg tablets), depending on your current phosphorus level.

• Standard Range: Most patients take between 3 and 6 tablets per day, divided among meals.

• Titration: Your doctor will check your blood every 2–4 weeks and may increase your dose by one tablet per meal until phosphorus is below 5.5 mg/dL.

• Doctor's Direction: Never skip doses, as phosphorus can rise very quickly.

If you forget to take your dose during a meal, and you have already finished eating, skip the missed dose. Do not take the dose between meals, as it will have no phosphorus to bind to and may only cause side effects. Resume your normal dose with your next meal.
