Nortriptyline is a tricyclic antidepressant that modulates pain perception by influencing key neurotransmitter pathways in the brain and spinal cord. Neuropathic pain arises from damage or dysfunction within the peripheral or central nervous system. Unlike pain caused by tissue injury, it originates from abnormal nerve signalling and may present as burning, tingling, shooting, or electric shock-like sensations. It is commonly associated with conditions such as diabetic neuropathy, postherpetic neuralgia, fibromyalgia, and nerve injury related to chemotherapy or spinal disorders.
Nortriptyline belongs to the tricyclic antidepressant (TCA) class and is used at lower doses for pain management than for depression. It may inhibit the reuptake of norepinephrine and serotonin in the central nervous system, enhancing descending pain inhibitory pathways that help suppress pain signals. It may also stabilise nerve membranes through sodium channel effects, contributing to its analgesic properties.
Pregabalin belongs to the gabapentinoid class and binds to the alpha-2-delta subunit of voltage-gated calcium channels in the nervous system. This reduces calcium influx into nerve terminals and decreases the release of excitatory neurotransmitters such as glutamate and substance P. The result is reduced neuronal hyperexcitability, a key contributor to neuropathic pain.
When used together, Nortriptyline and Pregabalin may provide broader and more sustained pain relief than either medicine alone. Both agents are well absorbed orally. Actanerv Nt 75 10mg Tablet 10s should be taken exactly as prescribed by your doctor. It is often taken once daily, typically in the evening, as its sedative effects may be better tolerated at night and may support sleep.
Treatment is usually started at a low dose and increased gradually to minimise side effects and improve tolerability. It may be taken with or without food. If stomach discomfort occurs, taking it with food may help.
Because neuropathic pain medicines act gradually, it may take several weeks to experience full benefit. Treatment adjustments should always be guided by a healthcare professional.
