Vesipress 50 Tablet is a cholinergic medication containing of Bethanechol chloride. It is primarily used to treat urinary retention (inability to urinate) that occurs after surgery, after giving birth, or due to other non-obstructive causes. Stimulating the parasympathetic nervous system helps the bladder muscle contract and relaxes the sphincter, allowing for the natural passage of urine. It is also occasionally used to help with certain digestive issues by increasing the motility of the stomach and intestines. Unlike other stimulants, Vesipress 50 Tablet specifically targets the "rest and digest" functions of the body to restore normal organ movement.
Vesipress 50 Tablet is a synthetic ester and a potent cholinergic agonist composed of Bethanechol. It is a critical therapeutic agent for restoring tone to the smooth muscles of the urinary and gastrointestinal tracts without significantly affecting the heart rate or blood pressure when taken at standard doses.
Composition and Mechanism: Each tablet contains of Bethanechol. The medication acts as a direct-acting muscarinic receptor agonist. It mimics the effects of acetylcholine, the primary neurotransmitter of the parasympathetic nervous system. When it binds to the muscarinic receptors in the bladder (detrusor muscle), it causes the muscle to contract while simultaneously relaxing the trigone and external sphincter muscles. This coordinated action increases the pressure within the bladder, overcomes the resistance of the outlet, and initiates micturition (urination).
Major Therapeutic Uses: Vesipress 50 Tablet is indicated for the treatment of acute postoperative and postpartum non-obstructive urinary retention. It is also used for neurogenic atony of the urinary bladder with retention, a condition where the nerves controlling the bladder do not function correctly. Beyond the bladder, it is sometimes utilised to treat gastric atony (slow stomach emptying) and as an alternative therapy for oesophageal reflux by increasing lower oesophageal sphincter pressure.
Benefits and Precautions: The primary benefit of Vesipress 50 Tablet is its ability to provide a non-invasive solution to urinary blockages, often preventing the need for painful catheterisation. However, significant precautions are required regarding physical obstructions. It must never be used if there is a physical blockage in the bladder neck or intestines (like a stone or tumour), as the forced contraction against a blockage could cause the organ to rupture. It must also be used with extreme caution in patients with asthma or hyperthyroidism, as it can trigger bronchial constriction or heart rhythm issues.
Medical Supervision and Risks: Treatment with Bethanechol requires careful observation by a healthcare professional. The drug’s effects usually begin within 30 to 90 minutes of oral administration. The most notable risks include "cholinergic crisis" symptoms if the dose is too high, such as extreme sweating, severe abdominal cramps, and a dangerous drop in blood pressure. It is generally not recommended for patients with a history of epilepsy, Parkinson’s disease, or active peptic ulcers, as it stimulates the production of stomach acid.

Alcohol can increase dizziness and the blood-pressure-lowering effects of this medication.

Store Vesipress 50 Tablet at room temperature (between 15°C and 30°C). Keep the tablets in their original container, tightly closed, and away from moisture. Protect the medicine from direct light and keep it out of reach of children.
It can lead to additive effects and a dangerous cholinergic overdose.
Atropine is the "antidote" and will completely block the effects of Bethanechol.
These heart medicines can interfere with the bladder-stimulating effects.
It can cause a severe and sudden drop in blood pressure.
If you miss a dose, take it as soon as you remember, as long as your stomach is empty. If it is almost time for your next dose, skip the missed one. Do not take two tablets at once to make up for a missed dose, as this significantly increases the risk of a "cholinergic crisis" (severe drop in heart rate and blood pressure).
