The combination works through complementary mechanisms to improve blood glucose control by reducing glucose production, improving insulin sensitivity, and enhancing glucose-dependent insulin release.

Tengreat M Tablet Sr is an oral anti-diabetic medication used to manage Type 2 Diabetes Mellitus in adults. It is a fixed-dose combination of Metformin, a biguanide, and Teneligliptin, a dipeptidyl peptidase-4 (DPP-4) inhibitor, which work together through complementary mechanisms to improve blood glucose control in people with Type 2 Diabetes Mellitus.

Metformin primarily lowers blood sugar by reducing the amount of glucose produced by the liver, improving the body’s sensitivity to insulin in muscle and fat tissues, and slightly decreasing glucose absorption from the gastrointestinal tract. Teneligliptin works by inhibiting the DPP-4 enzyme, which increases levels of incretin hormones. These hormones stimulate insulin release and reduce glucagon secretion in a glucose-dependent manner, helping to control blood sugar without causing excessive insulin release.

Because Teneligliptin enhances insulin secretion only when blood glucose levels are elevated, and Metformin does not stimulate insulin release, this combination has a relatively low risk of causing hypoglycaemia when used without other glucose-lowering medicines. The combination is valued for its ability to effectively reduce HbA1c while maintaining a generally weight-neutral profile and offering good tolerability.

Tengreat M Tablet Sr is usually taken with or after meals to help minimise gastrointestinal side effects commonly associated with Metformin, such as nausea, diarrhoea, and abdominal discomfort, which often improve over time. Regular monitoring of kidney function is recommended during treatment, as reduced renal function can increase the risk of lactic acidosis, a rare but serious adverse effect associated with Metformin. The medicine should be used under the guidance of a healthcare professional, who will determine the appropriate dose, formulation, and treatment duration based on individual patient needs.

Tengreat M Tablet Sr is indicated for:

Type 2 Diabetes Mellitus (T2DM):

Used as an adjunct to diet and exercise to improve glycemic control in adult patients who are inadequately controlled on Metformin or Teneligliptin monotherapy, or who are already being treated with both components as separate tablets.

Synergistic Efficacy:

Combines two complementary mechanisms. Metformin reduces glucose production in the liver and improves insulin sensitivity. Teneligliptin enhances insulin release in a glucose-dependent manner, resulting in better HbA1c reduction than either component alone.

Low Hypoglycemia Risk:

Teneligliptin increases insulin only when blood sugar is elevated, keeping the risk of low blood sugar low.

The combination generally does not cause weight gain.

Common Side Effects:

• Gastrointestinal disturbances (due to Metformin): Diarrhoea, nausea, vomiting, abdominal pain, and metallic taste. These often subside over time.
• Headache
• Upper respiratory tract infections (e.g., common cold symptoms)

Serious Side Effects (Stop taking the medicine and seek urgent medical attention if you experience):

• Lactic Acidosis (RARE but SEVERE): This is a medical emergency related to the Metformin component. Symptoms include deep and rapid breathing, severe muscle pain, severe tiredness, abdominal pain, and feeling unusually cold.
• Pancreatitis: Severe and persistent abdominal pain, with or without vomiting.
• Severe Allergic Reaction/Hypersensitivity: Swelling of the face, lips, tongue, or throat, leading to difficulty breathing or swallowing.

• Dosage: Take the tablet once or twice daily, as prescribed by your doctor.
• Administration: Swallow the tablet whole with a glass of water. It is recommended to take this medicine with or immediately after a meal to minimise the gastrointestinal side effects associated with Metformin.
• Monitoring: Regular monitoring of blood sugar, HbA1c, and especially kidney function (eGFR) is essential while on this medication.

• Hydration is Key: Drink plenty of water, especially if you have diarrhoea, vomiting, or fever, to reduce the risk of Lactic Acidosis.
• GI Issues: Nausea or diarrhoea is common initially. Always take the pill with food. If the issue is severe or persistent, contact your doctor.
• Scans/Surgery: Always inform medical staff that you are taking this medication before any procedure requiring a contrast agent or major surgery, as temporary discontinuation is required.
• Dual Check-ups: Regular blood tests are needed to check your blood sugar (for efficacy) and your kidney function (for safety).

Store the tablets in the original blister pack at room temperature (typically below 30 °C / 86 °F). Protect them from light and moisture. Keep all medications out of the reach of children.

• Food: Taking this combination with food is essential to managing GI side effects.

• Iodinated Contrast Agents: Tengreat M Tablet Sr should be temporarily stopped before and for 48 hours after any procedure, such as a CT scan, involving intravenous iodinated contrast dye. Kidney function should be checked before restarting the medication.
• Sulfonylureas or Insulin: Combining this medication with other potent blood sugar-lowering agents increases the risk of hypoglycemia. Dose adjustment of the other agents may be necessary.
• Cimetidine: May increase the concentration of Metformin in the blood.
• Corticosteroids, Thiazides, Thyroid Hormones: These can reduce the blood sugar-lowering effect of the combination, potentially requiring a dose increase.

• Renal Impairment: Contraindicated in severe renal impairment.
• Metabolic Acidosis (including Diabetic Ketoacidosis): Contraindicated.
• Pancreatitis: Should be discontinued if pancreatitis is suspected.
• Hypoxia/Acute Conditions: Should be temporarily stopped during conditions that can cause hypoxia (e.g., acute heart failure, severe infection) due to increased Lactic Acidosis risk.

The typical starting dose for this combination is often one tablet daily, with the maximum dose depending on individual response and tolerance, up to the maximum approved Teneligliptin dose of 20mg and the maximum tolerated Metformin dose.

Overdose is primarily associated with the risk of Lactic Acidosis (from Metformin) and, rarely, severe hypoglycemia (if combined with other anti-diabetics). Action Required: If an overdose is suspected, seek emergency medical attention immediately. Lactic Acidosis requires urgent treatment and specialised care.

If you miss a dose, take it as soon as you remember with food, unless it is almost time for your next dose. In that case, skip the missed dose and resume your regular schedule. Do not take a double dose to make up for the missed one.
