Jamcold Tablet

Jamcold Tablet is a four-component combination medicine that includes caffeine, ergotamine, paracetamol and phenylephrine, which is used for the symptomatic relief of common cold and flu. Paracetamol relieves pain and reduces fever, caffeine improves the effectiveness of paracetamol and counteracts drowsiness, chlorpheniramine maleate is a first-generation antihistamine that relieves a runny nose, sneezing, and watery eyes, and phenylephrine is a nasal decongestant that lowers congestion and stuffiness. Together, they treat multiple symptoms of cold & flu in a single formula.

Jamcold Tablet is a fixed-dose multi-symptom combination designed to provide broad relief from the overlapping symptoms of common cold, flu, and allergic rhinitis. Each component targets a specific symptom pathway, making the combination more convenient than taking separate medicines.

Paracetamol (acetaminophen) is the analgesic (pain-relieving) and antipyretic (fever-reducing) component, which stops the production of prostaglandin (compounds that raise body temperature) in the central nervous system to reduce fever and relieve body aches, headache, and sore throat. Caffeine enhances and prolongs the effect of paracetamol. It also provides mild stimulant activity, partially offsetting the sedation caused by chlorpheniramine. Chlorpheniramine maleate is a first-generation H1 antihistamine that blocks histamine (allergy-causing) receptors, reducing symptoms like a runny nose, watery eyes, sneezing, and nasal itching. Phenylephrine is a direct-acting alpha-1 adrenergic agonist that causes constriction of blood vessels of the nose, lowering swelling in the mucosa, congestion in the nose and sinus pressure.

Jamcold Tablet is intended for short-term use only and must not be used for more than 7 consecutive days, unless told otherwise by the doctor. It does not cure any viral infection.

Common cold and flu

Jamcold Tablet provides relief from symptoms of headache, body aches, fever, runny nose, nasal congestion, sneezing, watery eyes, and sore throat.

Allergic rhinitis

The anti-allergy and decongestant components of Jamcold Tablet help manage nasal and eye symptoms triggered by allergens such as pollen, dust, and pet dander.

Sinusitis

Phenylephrine helps lower nasal and sinus congestion, while paracetamol reduces the associated facial pain and headache.

• The relief may begin within 30 to 60 minutes for pain and fever, while the complete effect on nasal decongestion and allergy may take around 1 to 2 hours.

Common side effects of Jamcold Tablet may include

• Drowsiness or sedation

• Dry mouth (xerostomia)

• Restlessness

• Insomnia

• Nausea

• Headache

• Palpitations (rapid pounding of the heart)

• Tremors (shakiness)

• Difficulty urinating

Uncommon side effects of Jamcold Tablet may include

• Tachycardia (rapid heart rate)

• Increased blood pressure (hypertension)

• Anxiety

• Blurred vision

• Constipation

• Loss of appetite

• Lack of proper coordination

• Irritability

• Rebound nasal congestion, seen with long-term use of phenylephrine.

Serious side effects of Jamcold Tablet that may require immediate medical attention may include

• Severe allergic reaction, which may present as skin rash, angioedema (swelling of lips, throat or eyes) and breathing difficulty

• Damage to the liver due to paracetamol overdose or alcohol co-ingestion

• Cardiovascular symptoms such as severe hypertension, chest pain and irregular heartbeat

• Neurological symptoms such as hallucinations or seizures

Jamcold Tablet is available as oral tablets, capsules, or liquid. Swallow with a glass of water, with or without food. It must be noted that taking Jamcold Tablet with food reduces the chances of stomach upset.

While the standard adult dose (12 years and above) is 1 to 2 tablets or capsules, every 4–6 hours as needed. You should not exceed 4 doses in 24 hours. Jamcold Tablet should not be used for more than 7 consecutive days, or more than 3 days for fever alone. Do not take two medicines for a cold together, as most of them contain paracetamol, so this would increase your risk for accidental overdose.

When your body gets injured or infected, the cell membranes release a fatty acid called arachidonic acid. The COX enzyme converts that into prostaglandins. Prostaglandins are the chemicals that are responsible for pain, fever, swelling, and inflammation at the site. Paracetamol inhibits the cyclooxygenase (COX) pathway, reducing the production of prostaglandins, thereby lowering body temperature and decreasing pain perception.

The action of adenosine neurotransmitters slows down brain activity and makes you feel tired and sleepy. Caffeine stops the adenosine receptors and hence produces mild CNS stimulation. It also improves the pain-relieving property of paracetamol and partially reduces chlorpheniramine's sedative effects.

H1 receptors are the main histamine receptors that result in allergic reactions. Chlorpheniramine maleate competitively blocks H1 histamine receptors, preventing histamine from triggering a runny nose, mucosal swelling, sneezing, and watery eyes.

Phenylephrine stimulates alpha-1 adrenergic receptors that cause constriction of blood vessels of the nose, lowering swelling in the mucosa, nasal congestion and sinus pressure

• Never take two paracetamol-containing products at the same time. Check all cold, flu, pain, and sleep medicines for paracetamol (acetaminophen) before combining.

• Do not use for more than 7 days without medical advice. Seek medical attention if fever persists beyond 3 days or symptoms worsen.

• Avoid caffeine from food and drinks during treatment, as this medicine already contains caffeine.

• Do not give Jamcold Tablet to children under 12 years unless specifically recommended by a doctor.

• If you have high blood pressure, heart disease, diabetes, glaucoma, an enlarged prostate, or thyroid disease, consult your doctor before use.

• Do not take Jamcold Tablet within 14 days of stopping a monoamine oxidase inhibitor (MAOI) antidepressant.

Store at room temperature between 20–25°C, in a dry place away from direct sunlight and moisture. Do not freeze liquid formulations. Keep out of reach of children. Do not use beyond the expiry date.

• Caffeine-containing foods and drinks (MODERATE): Coffee, tea, cola, energy drinks, and chocolate add to the caffeine content of Jamcold Tablet. This increases the risk of insomnia, tremors, palpitations, and restlessness.

• Alcohol (SEVERE): Increases the risk of serious paracetamol-induced liver damage. Also increases the likelihood of chlorpheniramine's sedation, damaging judgment and coordination.

MAO inhibitors

SEVERE

Phenylephrine combined with MAOIs (phenelzine & linezolid) can trigger a life-threatening acute hypertensive crisis. Do not use within 14 days of stopping an MAOI.

Other paracetamol-containing medicines

SEVERE

Simultaneous intake of any other paracetamol-containing product heightens the risk of paracetamol overdose and liver toxicity.

Tricyclic antidepressants

CAUTION

Additive anticholinergic effects with chlorpheniramine and possible exaggeration of phenylephrine's cardiovascular effects.

CNS depressants (sedatives, opioids, sleep medicines)

CAUTION

Increased chances of chlorpheniramine's sedation, causing drowsiness & a high risk of accidents.

Beta-blockers

CAUTION

May heighten phenylephrine's blood vessel constriction effect, increasing the risk of severe hypertension.

Anticoagulants

CAUTION

Regular or high-dose paracetamol may increase warfarin's anticoagulant effect, increasing the risk of bleeding.

Other sympathomimetics

CAUTION

Combined with phenylephrine, they can increase the risk of high blood pressure & tachycardia.

• Liver disease or chronic alcohol use: Paracetamol is hepatotoxic at excess doses. This means continuous or overuse of paracetamol can heighten the risk of toxicity in people suffering from liver disease, alcoholism, or malnutrition.

• Cardiovascular diseases: Phenylephrine increases blood pressure and can worsen hypertension, angina (chest pain), and other cardiac conditions.

• Diabetes mellitus: Phenylephrine can increase blood glucose levels because some liquid formulations also contain sugar. Check the product labels before use.

• Narrow-angle glaucoma: Both chlorpheniramine and phenylephrine can both raise intraocular pressure, worsening narrow-angle glaucoma.

• Benign prostatic hyperplasia: People with an enlarged prostate can experience urinary retention due to Chlorpheniramine's anticholinergic activity. Anticholinergic activity involves blocking of acetylcholine, leading to involuntary muscle effects, dry mouth, constipation, urinary retention, and confusion.

• Hyperthyroidism: Phenylephrine's sympathomimetic effects (similar to fight or flight mode) may be exaggerated in people diagnosed with hyperthyroidism, increasing the risk of tachycardia (rapid heart rate) and hypertension (high blood pressure).

• Epilepsy or seizure disorders: High-dose chlorpheniramine may lower the seizure threshold, increasing the frequency of seizures.

• Severe renal impairment: Paracetamol dose adjustment may be required.

• Paracetamol overdose is the most dangerous risk and can be fatal even if the person feels completely alright in the beginning. Early signs include nausea, vomiting, loss of appetite, and abdominal pain. After 24 to 72 hours, the person may experience delayed symptoms such as right upper abdominal pain, dark urine, and jaundice, indicating liver damage.

• Phenylephrine overdose can cause severe hypertension and intense headache.

• Chlorpheniramine overdose may cause extreme sedation, hallucinations, and seizures.

• Caffeine overdose may cause palpitations, tremors, and agitation.

If overdose is suspected, consult a doctor immediately and do not wait for symptoms to develop with paracetamol overdose, as prompt treatment is essential to prevent irreversible liver damage.

Since Jamcold Tablet is typically taken as needed, a missed dose is usually not critical. If taken on a schedule, take the missed dose as soon as you remember, unless the next dose is due within 2 hours, then you can skip it. Never double the dose.
