These disorders occur when the immune system mistakenly attacks healthy joints, causing pain, swelling, stiffness, and progressive joint damage. Leflunomide works by suppressing specific immune cells responsible for inflammation and tissue destruction. Leflunomide may be used alone or in combination with other disease-modifying antirheumatic drugs (DMARDs) to slow disease progression and preserve joint function.
Dm Lef 20mg Tablet 10s contains leflunomide, an immunomodulatory agent classified as a disease-modifying antirheumatic drug (DMARD). It is primarily prescribed for adults with active rheumatoid arthritis and psoriatic arthritis to reduce inflammation, prevent joint damage, and improve long-term physical function.
Leflunomide works by inhibiting the mitochondrial enzyme dihydroorotate dehydrogenase (DHODH), which is essential for de novo pyrimidine synthesis. Activated lymphocytes require rapid DNA synthesis for proliferation. By blocking pyrimidine production, leflunomide suppresses the expansion of autoreactive T and B lymphocytes, thereby reducing immune-mediated inflammation.
Unlike simple pain relievers, leflunomide addresses the underlying disease process rather than just symptoms. Clinical improvement may begin within several weeks, but maximum therapeutic benefit often requires 6–12 weeks of consistent treatment. Early initiation can significantly reduce the risk of long-term joint deformity and disability.
Leflunomide is converted in the body to its active metabolite, teriflunomide, which has a prolonged half-life due to enterohepatic recirculation. The drug is metabolised in the liver and eliminated through both urine and bile. Because of its long half-life, drug elimination can take weeks unless a washout procedure is performed using cholestyramine. When prescribed appropriately and monitored regularly, Leflunomide is an effective long-term therapy for controlling autoimmune joint disease and preserving quality of life.
