Chlorthalidone is a long-acting thiazide-like diuretic that inhibits sodium and chloride reabsorption in the distal convoluted tubule of the kidney, increasing urinary sodium and water excretion. This reduces plasma volume and cardiac preload, lowering blood pressure.

Cilnidipine is a dual L-type and N-type calcium channel blocker. L-type blockade relaxes arterial vascular smooth muscle, reducing peripheral vascular resistance. N-type blockade additionally suppresses sympathetic noradrenaline release at nerve terminals, providing a more balanced antihypertensive effect with less reflex tachycardia compared to other dihydropyridine calcium channel blockers.

Olmesartan medoxomil is a prodrug that is converted to olmesartan after absorption.
