Budesonide is a synthetic steroid that belongs to the family of glucocorticosteroids. Due to its high anti-inflammatory property topically, it is often used to reduce inflammation in the airways, gut, and nasal passages. It is available in the form of inhalers, nasal sprays, tablets, capsules, & nebuliser solutions. It is often recommended to people diagnosed with conditions like asthma, chronic obstructive pulmonary disease (COPD), inflammatory bowel conditions (Crohn’s disease), and allergic rhinitis.

Budesonide belongs to the glucocorticoid class of drugs, which focuses on delivering its anti-inflammatory effects to the targeted areas, such as the airways, intestines, or nasal lining. They then break down rapidly in the liver, limiting their exposure to the rest of the body, making them safe for long-term use when compared to other systemic corticosteroids such as prednisolone or dexamethasone.

The function of Budesonide in the airways is to suppress immune cells, reduce mucus production, and reduce swelling in the bronchial walls, thus helping prevent asthma attacks and ease breathing in people diagnosed with COPD. In the gut, oral budesonide capsules release the drug in the ileum (the longest section of the small intestine) and the colon (the last section of the large intestine), reducing local inflammation responsible for Crohn's disease and microscopic colitis. The rectal foam formulation provides relief for ulcerative colitis, which affects the rectum and lower bowel.

It is advised to use Budeheal 0 5mg Respirator Suspension Respule 5x2ml under medical supervision as the dose, route of administration, and duration may vary based on your condition. It is not a quick-relief medicine for sudden attacks, as it often takes from days to weeks to gradually reduce baseline inflammation. Sudden discontinuation of Budeheal 0 5mg Respirator Suspension Respule 5x2ml after prolonged use should be avoided. Further, those switching from systemic steroids to budesonide should be monitored carefully for adrenal insufficiency. Adrenal insufficiency is a serious disorder where the adrenal glands (located on top of each kidney) stop producing enough cortisol (a steroid hormone), resulting in chronic fatigue, muscle weakness, weight loss and low blood pressure.

Budesonide may be prescribed by doctors for the following conditions

Asthma

Long-term prevention of asthma symptoms and attacks; available as an inhaler and nebuliser solution

COPD

To reduce exacerbations and airway inflammation, often combined with a long-acting bronchodilator

Crohn's disease

To induce remission in mild-to-moderate active Crohn's disease affecting the ileum and ascending colon

Ulcerative colitis

Budesonide in Budeheal 0 5mg Respirator Suspension Respule 5x2ml may:

• Target the site of inflammation, causing fewer systemic side effects compared to oral prednisone or dexamethasone

• Progressively lower inflammation in the airways, thus lowering the instances of asthma attacks and the need for rescue inhalers

• Help control symptoms and bring Crohn's disease and microscopic colitis to remission with fewer steroid-related complications

• Improve breathing through the nose and lowers allergy symptoms when used as a nasal spray

• Be used long-term in appropriate formulations after consulting with a licensed medical physician

• Be administered via multiple routes, including inhaled, oral, nasal or rectal, allowing targeted delivery

Common side effects of budesonide may include:

• Oral candidiasis (fungal infection in the mouth and throat) with inhaled forms

• Hoarseness or voice changes with inhalers

• Nausea, stomach discomfort, or bloating; seen with oral capsules

• Headache

• Nasal dryness or irritation; seen with nasal spray

• Mild upper respiratory infections

Uncommon side effects of budesonide may include:

• Skin bruising or thinning with prolonged use

• Mood changes or depression

• Sleep disturbances or restlessness

• Slower growth in children getting long-term inhaled budesonide

• High blood glucose levels

• Mild adrenal suppression (often seen with high-dose long-term use)

Serious side effects that may require immediate medical attention include:

• Paradoxical bronchospasm, sudden worsening of breathing after using the inhaler due to constriction of airways

• Severe adrenal insufficiency, particularly when switching from systemic steroids

• Hypersensitivity reactions, including rash, angioedema, or anaphylaxis

• Cushing's syndrome features, which include moon face, weight gain and purple coloured stretch marks with prolonged high doses

• Severe infections due to immune suppression

• Posterior subcapsular cataracts or raised intraocular pressure with long-term use

You must use Budeheal 0 5mg Respirator Suspension Respule 5x2ml exactly as prescribed by your doctor. Do not change the dose or frequency without medical advice.

• Inhaler: Shake well, exhale fully, seal your lips around the mouthpiece, inhale deeply, hold breath for 10 seconds. Always rinse your mouth with water and spit after each use to prevent oral fungal infection.

• Oral capsules: Swallow whole with water. Do not crush or chew. Take at the same time each day, usually in the morning before breakfast.

• Nasal spray: Blow your nose first. Tilt your head slightly forward, insert the tip into the nostril, and spray while inhaling gently through the nose. Use alternate nostrils as directed.

• Rectal foam:Follow the instructions provided with the applicator. Administer at bedtime and try to retain for as long as possible.

Do not stop using budesonide suddenly after prolonged use, as the dose may require tapering.

Budesonide acts through two major mechanisms: glucocorticoid receptor binding & gene regulation and immune cell suppression.

Budesonide binds to glucocorticoid receptors found inside the cells and forms a complex. This complex travels to the nucleus, where it stops pro-inflammatory genes (including cytokines and chemokines) that are responsible for inflammation. It further activates anti-inflammatory genes, reducing swelling, mucus production, and tissue inflammation.

Budesonide also reduces the activity of eosinophils, mast cells, and T-lymphocytes at the site of inflammation, thereby decreasing immune-mediated tissue damage.

• Always rinse your mouth and gargle with water after using a budesonide inhaler, as this would reduce the risk of oral candidiasis and hoarseness.

• Budesonide is a preventer, not a reliever. It will not stop a sudden asthma attack — always carry your rescue inhaler separately.

• If you are switching from systemic corticosteroids to budesonide, do not stop your previous steroid abruptly. Follow your doctor's tapering plan to avoid adrenal crisis.

• Avoid grapefruit juice throughout your treatment, as it can substantially increase budesonide levels in the blood.

• If you have diabetes, monitor your blood glucose more closely after starting budesonide, especially oral formulations.

• In children receiving long-term inhaled budesonide, regular monitoring of height is recommended, as high doses may slow down growth.

Store Budeheal 0 5mg Respirator Suspension Respule 5x2ml at room temperature (15–30°C) in a dry place away from direct sunlight and heat. Do not freeze the nebuliser solution or nasal spray. Keep inhalers away from open flames. Store oral capsules in their original packaging to protect from moisture. Keep all formulations out of the reach of children. Do not use after the expiry date printed on the packaging.

• Grapefruit and grapefruit juice (SEVERE): Grapefruit strongly inhibits CYP3A4 enzymes that break down budesonide in the gut wall and liver. This can increase budesonide blood levels significantly, increasing the risk of systemic steroid side effects.

• Alcohol (MODERATE): Alcohol consumption can worsen gastrointestinal inflammation, thereby reducing treatment effectiveness and increasing susceptibility to infections.

• Food duration (MILD): It is advised to take oral budesonide capsules on an empty stomach before breakfast to ensure consistent drug release in the intended part of the gut.

CYP3A4 inhibitors

SEVERE

Medications such as ketoconazole, itraconazole, ritonavir, and cobicistat are strong CYP3A4 inhibitors, an enzyme responsible for breaking down budesonide. This can lead to significantly high levels of budesonide and risk of systemic steroid toxicity, including adrenal suppression.

CYP3A4 inducers

CAUTION

Medications like rifampicin, phenytoin, and carbamazepine speed up budesonide metabolism, thereby reducing its effectiveness.

Antidiabetics

CAUTION

Antidiabetics like insulin, metformin, and sulfonylureas may increase blood glucose levels and interfere with glycaemic control.

Live vaccines

CAUTION

High-dose or prolonged corticosteroid therapy may impair immune response to live attenuated vaccines such as MMR and varicella.

Systemic Corticosteroids

CAUTION

Taking budesonide with systemic corticosteroids may increase the overall steroid burden, thereby raising the risk of adrenal suppression, Cushing's syndrome features, and metabolic side effects.

• Diabetes mellitus: Budesonide can raise blood glucose; hence, diabetics should monitor glucose levels more frequently when starting or changing doses.

• Active infections (TB, fungal, viral): Budesonide suppresses local immune responses and can worsen latent or active tuberculosis, systemic fungal infections, or serious viral infections. Screen for TB before starting treatment. Report any fever, night sweats, or unexplained weight loss to your doctor.

• Osteoporosis: Prolonged corticosteroid use, even with local formulations, can reduce bone density. Patients on long-term budesonide may benefit from calcium, vitamin D supplementation and periodic bone density assessment.

• Liver disease: Impaired hepatic metabolism reduces first-pass clearance of budesonide, increasing systemic exposure. Consulting a doctor is advised before starting the medication.

• Glaucoma or cataracts: Prolonged corticosteroid use can raise intraocular pressure and may worsen glaucoma. Regular eye check-ups are advised for patients on long-term treatment.

An acute single overdose of inhaled or oral budesonide is unlikely to cause a medical emergency. However, prolonged use at doses higher than prescribed may cause adrenal suppression, features of Cushing's syndrome, raised blood glucose, and immune suppression. If you suspect an overdose, stop using the medicine and contact your doctor immediately or go to the nearest emergency department. Bring the packaging with you so the treating team can assess the situation accurately.

If you miss a dose, take or use it as soon as you remember, unless it is almost time for your next dose. In that case, skip the missed dose and continue with your regular schedule. Never double the dose to compensate. For inhaled budesonide, missing a single dose will not cause immediate harm, but consistent daily use is important for the drug to work effectively.
