Budesonide is a synthetic steroid that belongs to the family of glucocorticosteroids. Due to its high anti-inflammatory property topically, it is often used to reduce inflammation in the airways, gut, and nasal passages. It is available in the form of inhalers, nasal sprays, tablets, capsules, & nebuliser solutions. It is often recommended to people diagnosed with conditions like asthma, chronic obstructive pulmonary disease (COPD), inflammatory bowel conditions (Crohn’s disease), and allergic rhinitis.
Budesonide belongs to the glucocorticoid class of drugs, which focuses on delivering its anti-inflammatory effects to the targeted areas, such as the airways, intestines, or nasal lining. They then break down rapidly in the liver, limiting their exposure to the rest of the body, making them safe for long-term use when compared to other systemic corticosteroids such as prednisolone or dexamethasone.
The function of Budesonide in the airways is to suppress immune cells, reduce mucus production, and reduce swelling in the bronchial walls, thus helping prevent asthma attacks and ease breathing in people diagnosed with COPD. In the gut, oral budesonide capsules release the drug in the ileum (the longest section of the small intestine) and the colon (the last section of the large intestine), reducing local inflammation responsible for Crohn's disease and microscopic colitis. The rectal foam formulation provides relief for ulcerative colitis, which affects the rectum and lower bowel.
It is advised to use Budeheal 0 5mg Respirator Suspension Respule 5x2ml under medical supervision as the dose, route of administration, and duration may vary based on your condition. It is not a quick-relief medicine for sudden attacks, as it often takes from days to weeks to gradually reduce baseline inflammation. Sudden discontinuation of Budeheal 0 5mg Respirator Suspension Respule 5x2ml after prolonged use should be avoided. Further, those switching from systemic steroids to budesonide should be monitored carefully for adrenal insufficiency. Adrenal insufficiency is a serious disorder where the adrenal glands (located on top of each kidney) stop producing enough cortisol (a steroid hormone), resulting in chronic fatigue, muscle weakness, weight loss and low blood pressure.
Budesonide may be prescribed by doctors for the following conditions

Do not stop using budesonide suddenly after prolonged use, as the dose may require tapering.
Budesonide acts through two major mechanisms: glucocorticoid receptor binding & gene regulation and immune cell suppression.
Budesonide binds to glucocorticoid receptors found inside the cells and forms a complex. This complex travels to the nucleus, where it stops pro-inflammatory genes (including cytokines and chemokines) that are responsible for inflammation. It further activates anti-inflammatory genes, reducing swelling, mucus production, and tissue inflammation.
Budesonide also reduces the activity of eosinophils, mast cells, and T-lymphocytes at the site of inflammation, thereby decreasing immune-mediated tissue damage.

Due to its low bioavailability and minimal absorption into breast milk, inhaled formulations of Budesonide are considered safe to use while breastfeeding. However, you may consult a doctor if you’re taking oral or any other form of Budesonide.
While Budesonide usually does not affect one's ability to drive, some people may experience dizziness. Hence, it is advised to drive only if you are alert.
Budesonide metabolises extensively in the liver. Therefore, people with significant hepatic impairment may have an increased risk of systemic exposure due to low clearance.
Before using Budesonide, talk to a doctor if you have severe renal impairment.
It is advised to completely refrain from or limit the consumption of alcohol if you’re taking Budesonide, as it can result in increased dizziness.
Avoid grapefruit and its juice, as they inhibit CYP3A4 and significantly increase budesonide blood levels, raising the risk of steroid side effects.
Store Budeheal 0 5mg Respirator Suspension Respule 5x2ml at room temperature (15–30°C) in a dry place away from direct sunlight and heat. Do not freeze the nebuliser solution or nasal spray. Keep inhalers away from open flames. Store oral capsules in their original packaging to protect from moisture. Keep all formulations out of the reach of children. Do not use after the expiry date printed on the packaging.
Medications such as ketoconazole, itraconazole, ritonavir, and cobicistat are strong CYP3A4 inhibitors, an enzyme responsible for breaking down budesonide. This can lead to significantly high levels of budesonide and risk of systemic steroid toxicity, including adrenal suppression.
Medications like rifampicin, phenytoin, and carbamazepine speed up budesonide metabolism, thereby reducing its effectiveness.
Antidiabetics like insulin, metformin, and sulfonylureas may increase blood glucose levels and interfere with glycaemic control.
High-dose or prolonged corticosteroid therapy may impair immune response to live attenuated vaccines such as MMR and varicella.
Taking budesonide with systemic corticosteroids may increase the overall steroid burden, thereby raising the risk of adrenal suppression, Cushing's syndrome features, and metabolic side effects.
An acute single overdose of inhaled or oral budesonide is unlikely to cause a medical emergency. However, prolonged use at doses higher than prescribed may cause adrenal suppression, features of Cushing's syndrome, raised blood glucose, and immune suppression. If you suspect an overdose, stop using the medicine and contact your doctor immediately or go to the nearest emergency department. Bring the packaging with you so the treating team can assess the situation accurately.
If you miss a dose, take or use it as soon as you remember, unless it is almost time for your next dose. In that case, skip the missed dose and continue with your regular schedule. Never double the dose to compensate. For inhaled budesonide, missing a single dose will not cause immediate harm, but consistent daily use is important for the drug to work effectively.
