Axacuf Syrup is a powerful, combined cough syrup designed to handle the problematic symptoms of wet, chesty coughs. These kinds of coughs often come with conditions like long-term bronchitis, asthma, or other chest infections where you have too much thick, sticky phlegm (mucus).

The syrup is special because it has four ingredients that work together:

Ambroxol: This is a mucus thinner. It works like a powerful cleaner, breaking down the structure of thick, gooey phlegm.

Guaifenesin: This is an expectorant. It makes the phlegm wetter and less sticky, making it much easier for you to cough up and clear out of your lungs.

Your doctor must start you on this treatment and determine the appropriate dose and duration. The dose is always tailored to you, based on your age, weight, and the severity of your breathing problem. The syrup form is usually chosen because it is easy to swallow and the medicine is absorbed quickly.

Axacuf Syrup is indicated for the symptomatic relief and management of respiratory conditions characterised by a productive cough (a cough that brings up mucus or phlegm) and bronchospasm (narrowing of the airways).

Bronchial Asthma:

Chronic Bronchitis:

Effective in liquefying and removing the copious, sticky mucus typical of this long-term condition.

• Ambroxol (Mucolytic): This component works by disrupting the internal structure of mucus, specifically by hydrolysing (breaking down) mucopolysaccharide fibres. This action effectively reduces the viscosity (thickness and stickiness) of the sputum, making it more fluid.

• Guaifenesin (Expectorant): Guaifenesin increases the volume of fluid in the respiratory tract. This action, known as reflux stimulation, lubricates the airways and thins the phlegm, facilitating the efficient function of the cilia to move the mucus out of the bronchial tubes (mucociliary clearance).

• Terbutaline (Bronchodilator): As a beta-2-adrenergic agonist, Terbutaline directly stimulates the beta-2-receptors found on the smooth muscle cells of the bronchi. This stimulation relaxes these muscles, widening the bronchial tubes. The result is reduced airway resistance and improved airflow.

• Menthol (Soothing Agent): Menthol is a volatile essential oil that acts as a mild local anaesthetic and counter-irritant on the mucous membranes of the throat and respiratory tract. This provides a comforting, cooling effect that helps to dampen the cough reflex, though it does not interfere with the productive action of the other ingredients.

The synergistic effect thins the mucus and clears it from the airways, opening them and providing effective relief from chest congestion and breathing difficulties.

• Hydration is Key: Drink plenty of fluids (water, clear broths). The Guaifenesin component works more effectively when the body is well-hydrated, as it helps thin mucus.

• Monitor Heart Rate: Due to the Terbutaline component, you might notice a slightly faster heart rate or mild tremors. If these symptoms become severe, consult your doctor immediately.

• Non-selective Beta-Blockers (e.g., Propranolol): These medications should generally be avoided, as they can block Terbutaline's bronchodilating effect, potentially leading to severe bronchospasm.

• Monoamine Oxidase Inhibitors (MAOIs) or Tricyclic Antidepressants (TCAs): Combining with Terbutaline can potentiate its cardiovascular effects, increasing the risk of arrhythmias, and should be done with extreme caution.

• Diuretics (Water Pills): Specifically, loop or thiazide diuretics, when taken with Terbutaline, may increase the risk of low potassium levels (hypokalaemia).

• Other Sympathomimetic Agents: Co-administration with other drugs that stimulate the sympathetic nervous system (like decongestants) can significantly increase the risk of adverse cardiovascular effects.

This medicine should be used with caution, or a dosage adjustment may be required, in patients with the following health problems:

• Heart Disease: Terbutaline can affect heart rhythm and increase heart rate. It must be used very carefully in patients with a history of heart attack, arrhythmias, or hypertension.

• Hyperthyroidism (Overactive Thyroid): The stimulant effects of Terbutaline can be amplified in patients with hyperthyroidism.

• Diabetes Mellitus: Terbutaline can potentially cause a temporary increase in blood glucose levels. Diabetic patients should closely monitor their blood sugar.

• Seizure Disorder: The Terbutaline component may lower the seizure threshold in susceptible individuals.

• The specific daily dosage for Axacuf Syrup is highly individualised and determined by the prescribing physician. It depends on the patient's age (adult or paediatric), weight, and the severity of the respiratory symptoms.

• The syrup is typically taken two to three times a day.

• Dosage is usually measured in millilitres (ml) using a standardised device.

• Never exceed the prescribed dose or the maximum daily recommended dose without consulting a healthcare provider.

An overdose of Axacuf Syrup is a medical emergency, primarily due to the effects of Terbutaline. Immediate medical attention is required.

Symptoms may be severe and include:

• Cardiovascular: Severe and persistent fast heart rate (tachycardia), palpitations, or chest pain.

• Neurological: Profound restlessness, severe headache, tremors, or seizures.

• Metabolic: Significant hypokalaemia (dangerously low potassium levels).

• If an overdose is suspected, seek emergency medical care without delay.

If a dose of Axacuf Syrup is missed:

• Take the missed dose as soon as you remember, unless it is almost time for the next scheduled dose.

• If it is nearly time for your next regular dose, skip the missed dose and continue with your regular schedule.

• Never take a double dose to compensate for the one you missed.
