Ogest 100mg Capsule is a high-potency synthetic progestogen medication containing of Hydroxyprogesterone Caproate. It is primarily used in specialised obstetric care to reduce the risk of preterm (premature) birth in pregnant women who have a clinical history of at least one spontaneous preterm delivery. Supplementing the body’s natural progesterone levels helps to maintain the uterine lining and keep the cervix closed and firm during the later stages of pregnancy.
Ogest 100mg Capsule belongs to the class of medications known as progestins. The dose is a long-acting esterified derivative of progesterone, designed for deep intramuscular administration to ensure a steady release of the hormone over an extended period.
Composition and Mechanism: Each 2ml or 5ml vial contains of Hydroxyprogesterone Caproate, usually dissolved in a castor oil or sesame oil base. It works by binding to progesterone receptors in the uterus (myometrium). During pregnancy, progesterone is responsible for maintaining "uterine quiescence"—it prevents the muscle from contracting prematurely. It also blunts the inflammatory signals that lead to the shortening and softening of the cervix. By providing a consistent external source of this hormone, Ogest 100mg Capsule reinforces the body's natural defences against early labour triggers.
Major Therapeutic Uses: Ogest 100mg Capsule is primarily indicated for the prevention of Preterm Birth in singleton pregnancies for women with a history of spontaneous preterm delivery. It is not intended for use in women with multiple gestations (twins or triplets). Additionally, it is used in gynaecology for the treatment of certain menstrual disorders, such as Amenorrhoea or Abnormal Uterine Bleeding caused by hormonal imbalance. In some oncology settings, high-dose hydroxyprogesterone may be used as a palliative treatment for Advanced Endometrial Cancer, where the hormone helps slow the growth of sensitive cancer cells.
Benefits and Precautions: The primary benefit of Ogest 100mg Capsule is extending gestational age, allowing the baby more time for vital lung and brain development in the womb. However, significant precautions are necessary regarding the injection site. Because of the oil-based carrier, local reactions like pain, swelling, and itching are common. Precautions are also vital for patients with a history of blood clots (thromboembolism), as high-dose progestins can slightly increase the risk of vascular issues. It should be used with caution in patients with pre-existing depression, as hormones can occasionally affect mood stability.
Medical Supervision and Risks: Use of Ogest 100mg Capsule must be strictly supervised by an obstetrician or a gynaecological specialist. Major risks include an increased risk of gestational diabetes and potential fluid retention. Patients should be monitored for signs of "preeclampsia" (high blood pressure) and changes in glucose tolerance. This medication is typically administered once weekly, starting around the 16th to 20th week of pregnancy and continuing until the 37th week or delivery.
Ogest 100mg Capsule (Hydroxyprogesterone 500mg) is indicated for:
Reducing the risk of early delivery in high-risk pregnancies.
Maintaining a quiet uterus during the second and third trimesters.
Palliative management of advanced uterine cancers.
Correcting amenorrhoea or excessive bleeding.
Part of specific hormone therapies for uterine health.
Ogest 100mg Capsule offers a "gestational-shield" approach to high-risk pregnancies. Its primary medicinal benefit is the chemical stabilisation of the uterus. For women who have previously experienced the trauma of a premature birth, the anxiety of a subsequent pregnancy can be high. The dose provides a reliable "safety net," ensuring that the levels of progesterone do not drop to a point that would trigger the body to begin labour too soon. This gives the foetus the best possible chance to grow in a stable environment.
One of the significant advantages of the long-acting injection is the convenience of weekly dosing. Unlike daily vaginal suppositories or oral pills, which can have fluctuating absorption rates, the deep intramuscular injection creates a "reservoir" of hormone that is slowly absorbed into the bloodstream. This ensures consistent 24-hour protection for the pregnancy, regardless of the patient's daily activity or digestive health. For many women, this consistent support is the key factor in reaching the 37-week milestone.
Furthermore, Ogest 100mg Capsule is designed to reduce the need for emergency interventions. By preventing early cervical shortening, it reduces the likelihood of needing a cervical cerclage (stitch) or of emergency hospital admissions for premature contractions.
Specifically used in pregnancy to prevent preterm birth. Use as directed by your OB-GYN.
Can raise blood sugar. Monitor your glucose levels closely if you have diabetes or are at risk.
AVOID if you have a history of active blood clots or pulmonary embolism.
Generally safe, but avoid driving if you feel dizzy or unusually sleepy after the injection.
Avoid smoking. Smoking during a high-risk pregnancy significantly increases the risk of preterm birth and blood clots.
Store Ogest 100mg Capsule at room temperature (15°C to 30°C). Do not refrigerate or freeze, as the oil base may thicken or crystallise. Protect the vial from direct light by keeping it in its original carton. Keep out of reach of children.
This antibiotic can speed up the breakdown of the hormone, making it less effective.
Certain epilepsy meds (like Phenytoin) may reduce the levels of hydroxyprogesterone.
Ogest 100mg Capsule may increase the levels of this immune-suppressing drug in the blood.
You may need an adjustment in your insulin or oral meds as hormones affect blood sugar.
The dosage of Ogest 100mg Capsule is strictly regulated for obstetric safety. For the Prevention of Preterm Birth, the standard dose is 500mg (usually 2ml) administered once weekly.
Treatment typically begins between Week 16 and Week 20 of pregnancy. The injections continue every seven days until Week 37 or until the baby is delivered. It is not a daily medication; the 500mg dose is designed to last for one full week. For gynaecological conditions like Amenorrhoea, a single 500mg dose may be given, or a different schedule may be followed as determined by your specialist. Always ensure a trained professional performs the injection.
If you miss your scheduled weekly injection, contact your doctor immediately to reschedule. Do not wait until the following week. Timely administration is critical for maintaining the pregnancy. Do not take a double dose to catch up, as this can cause significant hormonal side effects.
