Terbilet 250 Tablet is a potent allylic amine antifungal medication used to treat a variety of fungal infections affecting the skin and nails. It is highly effective against dermatophytes, which are fungi that thrive on keratin in areas like the hair, skin, and nails. Common conditions treated include athlete’s foot, ringworm, jock itch, and fungal nail infections (onychomycosis). By interfering with the early stages of fungal sterol synthesis, it leads to the death of the fungal cells. It is available in various formulations, including oral tablets, topical creams, gels, and sprays.
Terbilet 250 Tablet contains terbinafine, a broad-spectrum antifungal agent that is particularly effective against dermatophytes, as well as certain yeasts and moulds. It is used to manage both superficial skin infections and more deep-seated nail infections that do not respond well to topical treatments alone.
The medication works by inhibiting an enzyme called squalene epoxidase in the fungal cell membrane. This inhibition creates a dual effect: it prevents the formation of ergosterol (an essential component of the fungal cell wall) and causes an accumulation of squalene within the fungal cell. The high levels of squalene are toxic to the fungus, resulting in its rapid death. Because it targets an early step in the fungal metabolic pathway, it is often more effective at killing fungi (fungicidal) rather than just stopping their growth (fungistatic).
When taken orally, terbinafine is highly lipophilic and keratophilic, meaning it concentrates in the skin, hair, and nails, providing long-lasting protection even after the treatment course is finished. Topical forms provide localised relief for skin infections by penetrating the upper layers of the skin. Treatment duration varies significantly, ranging from one week for simple skin infections to several months for nail infections. It should be used under medical supervision to monitor for potential impact on liver health (particularly in oral forms).

Terbilet 250 Tablet works by disrupting the formation of the fungal cell membrane. It inhibits an enzyme called squalene epoxidase, which is responsible for the first step in creating ergosterol. This leads to two critical changes: first, it results in a deficiency of ergosterol, which weakens the fungal cell wall; second, it causes a buildup of a substance called squalene inside the fungus. Because humans do not use the same enzyme pathway for sterol synthesis, the medicine specifically targets the fungi while leaving human cells unaffected.
