Detailed Description

Amzak 5mg Tablet 10s (amlodipine) is a long-acting dihydropyridine calcium channel blocker used to treat high blood pressure (hypertension) and angina (chest pain). It works by blocking calcium entry into the smooth muscle cells of blood vessels, which causes the vessels to relax and widen. This lowers blood pressure and reduces the workload on the heart.

In hypertension, amlodipine primarily acts on peripheral arteries, decreasing vascular resistance and improving blood flow. Its gradual onset and long duration of action provide stable, 24-hour blood pressure control with once-daily dosing, helping to reduce day-to-day blood pressure fluctuations.

In angina, amlodipine improves blood supply to the heart by dilating the coronary arteries and preventing coronary artery spasm. This increases oxygen delivery to the heart muscle and helps reduce the frequency and severity of chest pain. It is effective in both chronic stable angina and vasospastic (Prinzmetal’s) angina.

Amlodipine may be used alone or in combination with other cardiovascular medicines, such as ACE inhibitors, beta blockers, or diuretics. It is generally well tolerated, with common side effects related to vasodilation, such as ankle swelling, flushing, or headache. Because of its proven effectiveness, safety profile, and convenient once-daily dosing, Amzak 5mg Tablet 10s is widely used for long-term cardiovascular management.

Uses of Amzak 5mg Tablet 10s

Amzak 5mg Tablet 10s is indicated for:
Hypertension (High Blood Pressure):

o relieve symptoms of chest pain associated with physical exertion.

Vasospastic Angina (Prinzmetal's Angina):

To prevent episodic, recurrent chest pain caused by temporary spasms of the coronary arteries.

Benefits of Amzak 5mg Tablet 10s

24-Hour Efficacy:

Provides consistent, prolonged blood pressure control due to its long half-life.

Cardioprotective:

Reduces the workload on the heart and improves oxygen supply to the cardiac muscle, beneficial for angina patients.

Once-Daily Dosing:

Simplifies treatment regimens and improves patient adherence.

Broad Applicability:

Effective across various patient populations, including the elderly and those with diabetes or chronic kidney disease.

Side Effects of Amzak 5mg Tablet 10s

Amzak 5mg Tablet 10s is generally well-tolerated, but some side effects may occur.

Common Side Effects (Usually Mild and Dose-Dependent):
  • Peripheral oedema (swelling of the ankles and feet) - The most common side effect.
  • Headache
  • Fatigue/Drowsiness
  • Nausea
  • Flushing (warmth or redness in the face or neck)
Serious Side Effects (Seek urgent medical attention if you experience):
  • Severe Hypotension: Extreme dizziness, lightheadedness, or fainting, indicating dangerously low blood pressure.
  • Worsening Angina or Heart Attack: Chest pain that is new, severe, or occurring more frequently.
  • Severe Allergic Reaction: Rash, itching, severe dizziness, trouble breathing, or swelling of the face, tongue, or throat.
  • Hepatitis or Jaundice: Yellowing of the skin or eyes.

Directions for Use

  • Dosage: The starting dose is usually 5mg once daily. This may be increased to a maximum of 10mg once daily if the clinical response is insufficient.
  • Administration: Take the tablet orally, once daily, at approximately the same time each day. It can be taken with or without food.
  • Consistency: Take Amzak 5mg Tablet 10s every day, even if you feel well. High blood pressure often has no symptoms.
  • Swallowing: Swallow the tablet whole with water.

How it works

Amzak 5mg Tablet 10s is a peripheral arterial vasodilator that acts directly on vascular smooth muscle.

Calcium Channel Blockade: It selectively blocks the entry of calcium ions (Ca2+) into vascular smooth muscle and cardiac muscle cells. Specifically, it acts on the L-type calcium channels.

Vasodilation: By limiting the influx of calcium, Amzak 5mg Tablet 10s inhibits the contraction of arterial smooth muscles. This causes the arteries throughout the body (peripheral arteries) to relax and widen (vasodilation).

Blood Pressure Reduction: The widening of arteries reduces the total peripheral vascular resistance (the resistance the heart pumps against). This reduction effectively lowers systemic blood pressure.

Angina Relief: In the heart, it causes coronary vasodilation (widening of the arteries supplying the heart), increasing oxygen delivery. It also reduces peripheral resistance, decreasing the heart's workload and oxygen demand, thereby preventing and treating angina.

Safety Advice for Amzak 5mg Tablet 10s

icon
Pregnancy
Caution

Use only if the potential benefit outweighs the potential risk to the fetus (Category C). Consult a doctor immediately if planning a pregnancy or if you become pregnant.

icon
Breastfeeding
Caution

Small amounts pass into breast milk. Use only if advised by a healthcare professional; monitor the infant for side effects.

icon
Driving
Caution

May cause dizziness, drowsiness, or fatigue, especially when first starting treatment or increasing the dose. Do not drive or operate heavy machinery until you know how the medication affects you.

icon
Liver
Caution

Metabolised extensively by the liver. Use with caution in patients with severe hepatic impairment; a lower starting dose (2.5mg) may be required.

icon
Kidney
Safe

No dose adjustment is typically needed for patients with renal impairment or those on dialysis, as it is primarily metabolised to inactive compounds.

icon
Alcohol
Caution

Alcohol can increase the blood pressure-lowering effects, leading to excessive dizziness or lightheadedness. Limit or avoid alcohol consumption.

Quick Tips for Amzak 5mg Tablet 10s

  • Manage Swelling: Mild ankle or foot swelling (oedema) is common with Amzak 5mg Tablet 10s. Elevate your legs when resting and stay lightly active. Contact your doctor if swelling becomes severe, painful, or sudden.
  • Take at the Same Time Daily: Taking your medicine at the same time each day (often in the morning) helps maintain consistent blood pressure control.
  • Monitor Your Blood Pressure: Check your blood pressure regularly at home and keep a record to share with your healthcare provider. This helps track how well the medicine is working.
  • Stand Up Slowly: Dizziness can occur, especially when you first start the medicine. Rise slowly from sitting or lying down to reduce the risk of lightheadedness from temporary low blood pressure.
  • Report Unusual Symptoms: Contact your doctor if you experience severe headache, palpitations, chest pain, or sudden swelling, as these may require medical attention.

Storage Advice

  • Store at room temperature, ideally between 20°C and 25°C (68°F and 77°F).
  • Keep the tablets in their original container, protected from excessive moisture.
  • Keep out of reach and sight of children.

Drug-Food Interaction

  • Grapefruit Juice: Although the interaction is generally considered minor and less significant than with some other CCBs, some reports suggest grapefruit juice might slightly increase Amzak 5mg Tablet 10s levels. It is generally safe to consume in moderation, but avoid large quantities.
  • General: No other significant food interactions are known.

Interactions with Other Drugs

  • Strong CYP3A4 Inhibitors (e.g., Ketoconazole, Ritonavir, Clarithromycin): These drugs can significantly increase the exposure to Amzak 5mg Tablet 10s, potentially leading to excessive blood pressure lowering (hypotension) or oedema. Dose adjustment may be needed.
  • Strong CYP3A4 Inducers (e.g., Rifampin, St. John's Wort): These drugs can decrease the plasma concentration of Amzak 5mg Tablet 10s, potentially reducing its effectiveness.
  • Simvastatin: High doses of Simvastatin (above 20mg) should be avoided when taking Amzak 5mg Tablet 10s, as it increases the risk of muscle problems (myopathy/rhabdomyolysis) from the statin.

Drug-Disease Interactions

  • Severe Hypotension: Must be used with caution in patients who already have severely low blood pressure.
  • Congestive Heart Failure (CHF): Use with caution, particularly in patients with severe obstructive coronary artery disease or severe aortic stenosis.
  • Hepatic Impairment (Liver Disease): Metabolism is reduced, leading to longer half-life and higher blood levels. A lower starting dose (2.5mg) is recommended for severe liver disease.

Daily Dose

The typical starting daily dose is 5mg taken once daily. The dose can be adjusted by a healthcare professional up to a maximum of 10mg once daily.

Overdose

In the event of an overdose, the main effect is pronounced peripheral vasodilation leading to severe and prolonged systemic hypotension and possibly reflex tachycardia (rapid heart rate). Action Required: Seek immediate emergency medical attention. Treatment is supportive, focusing on cardiovascular maintenance, including active monitoring, possible gastric lavage, and use of intravenous fluids and vasopressors if necessary.

What If You Forget to take Amzak 5mg Tablet 10s?

If you miss a dose, take it as soon as you remember unless it is almost time for your next dose (more than 12 hours late). If it is close to the next dose, skip the missed one and continue your regular schedule. Do not take two doses at once.
