Tolvaptan is a specialised prescription medication containing of Tolvaptan. It belongs to a class of drugs known as selective vasopressin V2-receptor antagonists (vaptans). It is primarily used to treat hyponatremia (low blood sodium levels) associated with conditions like heart failure and the Syndrome of Inappropriate Antidiuretic Hormone (SIADH). By promoting "aquaresis", the excretion of excess water without losing electrolytes, it helps restore a healthy balance of sodium in the blood.

Tolvaptan is an oral selective vasopressin antagonist composed of Tolvaptan. It is a breakthrough therapy for managing fluid balance in patients whose bodies retain too much water, leading to dangerously diluted sodium levels.

Composition and Mechanism: Each tablet contains of Tolvaptan. The medication works by selectively blocking the binding of the antidiuretic hormone (vasopressin) to the V2-receptors located in the collecting ducts of the kidneys. Normally, vasopressin tells the kidneys to reabsorb water back into the bloodstream. By blocking this signal, Tolvaptan causes the kidneys to excrete large amounts of dilute urine. Unlike traditional "water pills" (diuretics), it removes water without significantly affecting the excretion of sodium or potassium.

Major Therapeutic Uses: Tolvaptan is indicated for the treatment of clinically significant hypervolemic and euvolemic hyponatremia. This includes patients with heart failure or SIADH who have not responded to fluid restriction. It is also used in higher, specific dosing regimens to slow the progression of kidney cyst formation in patients with Autosomal Dominant Polycystic Kidney Disease (ADPKD).

• Dizziness: Feeling lightheaded, especially during the first few days.

• Loss of Appetite: Reduced desire to eat.

Serious Side Effects (Requires immediate medical attention):

• Liver Injury: Symptoms include unusual tiredness, nausea, jaundice (yellowing of eyes/skin), or right-sided abdominal pain.

• Osmotic Demyelination Syndrome: Difficulty speaking, swallowing, or moving limbs; sudden confusion or seizures.

• Severe Dehydration: Extreme thirst, sunken eyes, or fainting.

• Rapid Heartbeat: Palpitations accompany severe fluid loss.

• Timing: Usually taken once daily, preferably in the morning to avoid multiple nighttime trips to the bathroom.
• Method: Swallow the tablet whole with a glass of water. It can be taken with or without food.
• Water Access: You must have access to water at all times. Drink whenever you feel thirsty.
• Hospital Initiation: Your first several doses will be given in a hospital to monitor your blood sodium levels closely.
• Duration: For hyponatremia, Tolvaptan is typically not used for longer than 30 days.

• Drink When Thirsty: This is the most important rule. Your body's thirst mechanism is the best defence against sodium levels rising too fast.

• Monitor Liver Health: Your doctor will perform blood tests for your liver before and during treatment. Report any dark urine or yellowing of the skin immediately.

• No Fluid Restriction: While most hyponatremia patients are told to limit fluids, you must stop fluid restriction once you start Tolvaptan.

• Hospital Follow-up: Ensure you attend all scheduled blood tests to check your sodium levels.

Store Tolvaptan at room temperature (25°C). Keep the tablets in their original light-resistant container and protect them from moisture. Keep out of reach of children.

• Grapefruit Juice: Can significantly increase the concentration of Tolvaptan in your blood, increasing the risk of side effects.

• St. John’s Wort: Can reduce the effectiveness of Tolvaptan.

Ketoconazole / Clarithromycin

Strong CYP3A inhibitors can cause Tolvaptan levels to skyrocket.

Rifampin / Phenytoin

These can lower Tolvaptan levels, making the 15mg dose less effective.

Desmopressin (dDAVP)

Works in direct opposition to Tolvaptan; can interfere with sodium correction.

Hypertonic Saline

CAUTION

Combining with Tolvaptan can cause sodium to rise dangerously fast.

• Liver Disease: Use is contraindicated in patients with underlying liver disease or cirrhosis, as the risk of liver failure is higher.

• Inability to Sense Thirst: Patients who cannot perceive thirst or cannot respond to thirst by drinking water should not take this medication.

• Urinary Obstruction: If you cannot urinate at all, this medicine will not work and should not be used.

• Initial Dose: Usually 15mg once daily.

• Titration: After 24 hours, the doctor may increase the dose to 30mg or 60mg if sodium levels haven't improved sufficiently.

• ADPKD Dosing: Often involves a split-dose regimen (e.g., 45mg in the morning and 15mg in the afternoon), but this is a specialised protocol.

• Doctor's Direction: Never stop or start this medication without hospital-level monitoring.

If you miss a dose, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed one. Do not take two 15mg tablets at once. If you miss a dose for more than a day, contact your doctor as your sodium levels may fluctuate.
