By blocking this protein, Ezetimibe reduces the amount of cholesterol that enters your bloodstream from the intestine.

As a result of this reduced absorption, the liver senses a shortage of cholesterol and responds by pulling more LDL-C out of the blood. This process helps to further lower the levels of circulating “bad cholesterol.” Unlike statins, which work by slowing down cholesterol production in the liver, Ezetimibe focuses on limiting how much cholesterol is absorbed from the digestive tract. This complementary mechanism makes it a valuable option in cholesterol management.

Ezetimibe is typically taken once daily, with or without food. For best results, it should be used as part of a comprehensive treatment plan that includes a heart-healthy diet, regular exercise, and lifestyle changes such as quitting smoking and maintaining a healthy weight. Ezetimibe is primarily used to manage hypercholesterolemia (high cholesterol) in adults.

Primary Hypercholesterolemia:

Used alone or in combination with an HMG-CoA reductase inhibitor (statin) to lower elevated total cholesterol, LDL-C, and ApoB (Apolipoprotein B).

Homogenous Familial Hypercholesterolemia (HoFH):

Used in combination with a statin to reduce elevated cholesterol levels.

Sitosterolemia (Phytosterolemia):

A rare genetic disorder characterised by high plant sterol levels; Ezetimibe helps reduce these levels.

Atherosclerotic Cardiovascular Disease (ASCVD) Risk Reduction:

Used in combination with a statin to reduce the risk of major cardiovascular events, such as heart attack and stroke.

Provides a significant reduction in LDL-C (bad cholesterol), which is a key factor in the development of atherosclerosis (hardening of the arteries).

Serious Side Effects (Seek immediate medical attention if you experience):

• Unexplained Muscle Pain/Weakness: Especially if accompanied by fever or dark urine, as this can be a sign of a serious condition called rhabdomyolysis (rare, usually when combined with a statin).

• Allergic Reaction: Swelling of the face, lips, tongue, or throat; difficulty breathing; rash or hives.

• Liver Problems: Unexplained fatigue, loss of appetite, pain in the upper right abdomen, dark urine, or yellowing of the skin or eyes (jaundice).

Always take Ezetimibe exactly as prescribed by your doctor.

• Swallow the tablet whole with a glass of water. Do not crush, chew, or break it.

• Take the medicine once daily at the same time each day to maintain consistent levels in your body.

• It can be taken with or without food.

• If you are also taking a bile acid sequestrant (like cholestyramine), take Ezetimibe at least 2 hours before or 4 hours after taking the sequestrant.

• Continue to follow your cholesterol-lowering diet and exercise plan as recommended by your doctor while taking this medication.

Ezetimibe achieves its effect by selectively inhibiting the transport of cholesterol across the wall of the small intestine.

• Target Protein: Ezetimibe targets the Niemann-Pick C1-Like 1 (NPC1L1) protein, which is essential for cholesterol uptake by the enterocytes.

• Inhibition: By binding to and inhibiting the NPC1L1 protein, Ezetimibe prevents the absorption of cholesterol originating from both ingested food and biliary secretions.

• Hepatic Response: The reduction of cholesterol delivery to the liver leads to an increase in the number of LDL receptors on liver cells.

• Clearance: This upregulation of LDL receptors enhances the clearance of LDL-C from the blood, resulting in lower plasma cholesterol concentrations.

• Routine Testing is Key: Your doctor will need to check your liver function (LFTs) and lipid profile periodically (usually every 3-6 months) to ensure the medication is working safely and effectively.

• Consistency Matters: Take the tablet at the same time every day to establish a routine. The timing (morning or evening) is not critical, but consistency is.

• Focus on Diet: Remember that Ezetimibe only blocks absorption; it doesn't stop synthesis. A heart-healthy diet remains the foundation of cholesterol management.

• Report Muscle Pain: If you experience any unexplained muscle pain, tenderness, or weakness, stop taking the medicine (especially if on a statin combination) and call your doctor immediately.

• Store the tablets at room temperature, ideally between 15°C to 30°C or 59°F to 86°F.

• Keep the medicine away from excessive moisture and heat.

• Keep the tablets out of the reach of children and pets.

• Do not use the medication past the expiry date printed on the packaging.

There are no known significant food interactions with Ezetimibe. It can be taken with or without food.

The following drugs may interact with Ezetimibe:

• Statins (e.g., Atorvastatin, Simvastatin): This is a common and usually beneficial combination, but it requires close monitoring for liver and muscle-related side effects.

• Fibrates (e.g., Fenofibrate, Gemfibrozil): Using Ezetimibe with Gemfibrozil is generally not recommended, as it may increase the risk of gallstones and increase Ezetimibe levels.

• Cyclosporine: Co-administration significantly increases the exposure of Ezetimibe. Dose adjustment and close monitoring are required.

• Bile Acid Sequestrants (e.g., Cholestyramine, Colestipol): These medications significantly reduce the absorption of Ezetimibe. Ezetimibe must be taken at least 2 hours before or 4 hours after taking a bile acid sequestrant.

• Warfarin or other Coumarin Anticoagulants: Close monitoring of INR is required when starting or stopping Ezetimibe, as it may affect the anticoagulant's effectiveness.

Patients with certain pre-existing conditions need caution:

• Liver Disease: Ezetimibe is not recommended for patients with moderate to severe liver disease. For patients taking it in combination with a statin, it is contraindicated in active liver disease or unexplained, persistent elevation of liver enzymes.

• Kidney Disease: No significant dose adjustment is typically needed for kidney impairment, but caution is advised in severe cases.

• Gallbladder Disease: Caution is needed if you are taking a Fibrate (like Fenofibrate) in combination with Ezetimibe due to an increased risk of gallstones (cholelithiasis).

The recommended dose of Ezetimibe is 10 mg taken once daily, with or without food. This applies whether it is used alone (monotherapy) or in combination with a statin to help lower cholesterol levels.

For best results, take Ezetimibe at the same time each day, and follow your doctor’s instructions closely. Do not adjust the dose or stop the medication without medical advice.

If an overdose of Ezetimibe is suspected, seek immediate medical attention or contact your healthcare provider without delay. While serious complications are rare, especially when Ezetimibe is taken alone, appropriate clinical evaluation and supportive care are essential.

If you miss a dose of Ezetimibe, take it as soon as you remember. However, if it is almost time for your next scheduled dose, skip the missed dose and continue with your regular dosing schedule. Do not take two doses at the same time to catch up. Consistent, daily use is essential for optimal cholesterol management.
