Rybelsus is the first and only oral formulation of semaglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist, taken once daily as a tablet. It is approved by the U.S. Food and Drug Administration as an adjunct to diet and exercise to improve glycaemic control in adults with type 2 diabetes mellitus, and to reduce the risk of major adverse cardiovascular events in adults with type 2 diabetes and established cardiovascular disease. Weight loss is consistently observed during treatment, but it is not a primary indication for Rybelsus.

How Does Rybelsus Work?

Rybelsus represents a pharmaceutical advance because semaglutide is a peptide, and peptides are typically destroyed in the stomach before absorption. The Rybelsus tablet co-formulates semaglutide with an absorption enhancer, sodium N-(8-[2-hydroxybenzoyl] amino) caprylate (SNAC), which raises local gastric pH and protects the peptide from enzymatic degradation, allowing intact semaglutide to cross the stomach lining. This is what makes Rybelsus the only oral GLP-1 receptor agonist available.

Once absorbed, semaglutide acts on GLP-1 receptors throughout the body. In the pancreas, it enhances glucose-dependent insulin secretion and suppresses glucagon, producing glucose lowering without a high baseline risk of hypoglycaemia. Rybelsus has two approved uses in adults with type 2 diabetes mellitus, with specific exclusions:

  • Cardiovascular risk reduction: To reduce the risk of major adverse cardiovascular events (including cardiovascular death, non-fatal myocardial infarction, and non-fatal stroke) in adults with type 2 diabetes and established cardiovascular disease.
  • Not approved for: Type 1 diabetes, diabetic ketoacidosis, or chronic weight management.
  • Off-label note: Weight loss is commonly experienced during treatment, but prescribing Rybelsus specifically for weight loss in someone without type 2 diabetes is off-label. The clinical evidence for semaglutide in obesity comes from trials of the injectable formulation Wegovy.

What Is the Dosage of Rybelsus?

Rybelsus must be taken once daily on an empty stomach, immediately after waking, with no more than 120 millilitres of plain water. After taking the tablet, the patient must wait at least 30 minutes before eating, drinking anything other than plain water, or taking any other oral medication. Tablets must be swallowed whole and not split, crushed, or chewed.

Treatment begins at 3 mg once daily for 30 days. The 3 mg dose is intended to improve gastrointestinal tolerability and is not effective for glycaemic control on its own. After 30 days, the dose is increased once daily. If further glycaemic control is required after at least another 30 days, the dose may be increased to a maximum of 14 mg once daily. Tablets should be stored in the original blister pack and protected from moisture.

What Are the Side Effects of Rybelsus?

The most frequent side effects are gastrointestinal and tend to be most prominent when the dose is increased. This usually diminishes over time. These include:

  • Gastrointestinal: Nausea, abdominal pain, diarrhoea, decreased appetite, vomiting, constipation, dyspepsia (indigestion), belching, and flatulence may also occur.
  • Delayed gastric emptying effects: Some patients experience fullness or early satiety.
  • General: Headache and fatigue.

Serious reactions that require prompt medical attention:

  • Acute pancreatitis: Presents as severe, persistent abdominal pain that may radiate to the back.
  • Acute gallbladder disease: Including cholelithiasis and cholecystitis, particularly with rapid or substantial weight loss.
  • Acute kidney injury: Generally arises from dehydration secondary to gastrointestinal effects.
  • Hypoglycaemia: Uncommon with Rybelsus monotherapy, but more likely when combined with insulin or sulfonylureas.
  • Hypersensitivity reactions: Anaphylaxis and angioedema are rare but possible.
  • Worsening of diabetic retinopathy: Observed during rapid glycaemic improvement.
  • Pulmonary aspiration: Reported in patients undergoing procedures requiring sedation or general anaesthesia

What Are the Safety Guidelines for Rybelsus?

Rybelsus carries a boxed warning regarding the risk of thyroid C-cell tumours observed in rodent studies, however, the human relevance has not been established.

Contraindications

  • Personal or family history of medullary thyroid carcinoma
  • Multiple Endocrine Neoplasia syndrome type 2
  • Prior serious hypersensitivity reaction to semaglutide
  • Pregnancy or if planning for pregnancy (discontinue at least two months before a planned pregnancy)

Use with caution in

  • History of pancreatitis (discontinue if pancreatitis is suspected)
  • Severe gastrointestinal disease or gastroparesis
  • Diabetic retinopathy
  • Renal impairment
  • Planning on undergoing elective procedures with sedation or general anaesthesia (it can delay gastric emptying, increasing the risk of pulmonary aspiration)

Drug interactions

  • Insulin (SEVERE): Risk of hypoglycaemia (low blood sugar levels). Insulin dose reductions may be required when the treatment is started.
  • Sulfonylureas (SEVERE): Increased hypoglycaemia risk; dose reduction often necessary.
  • Other semaglutide-containing products or GLP-1 receptor agonists (SEVERE): No added efficacy and raised adverse-effect risk; must not be combined.
  • Warfarin and other narrow-therapeutic-index oral medications (CAUTION): Absorption may be altered by delayed gastric emptying; monitor clinical effect or relevant laboratory values.
  • Levothyroxine (CAUTION): Rybelsus increases levothyroxine exposure. Monitor thyroid function and adjust dose if needed.

Food and administration rules

Food is the most important interaction with Rybelsus:

  • Wait at least 30 minutes before eating, drinking anything other than plain water, or taking any other oral medication.
  • Larger water volumes or eating sooner substantially reduce absorption and reduce efficacy.

Missed doses

  • If a dose is missed, skip it and take the next dose at the regular time the following day.
  • A second dose should never be taken on the same day to make up for a missed one.
  • Do not double the next day's dose.

Overdose

Overdose may cause prolonged severe gastrointestinal symptoms. The treatment may include supportive care with attention to hydration. However, symptoms may persist for several days, given the long half-life of semaglutide. In such cases, patients should seek emergency care.
