Lactobacillus Rhamnosus is a gram-positive, probiotic bacterium that is supplied as capsules, sachets, chewable tablets, drops, and fortified foods such as yoghurts and fermented milks.

Because Lactobacillus Rhamnosus can survive stomach acid and bile, it passes through the stomach and reaches the small intestine and colon while still active. Once it reaches the small intestine, it sticks to the gut lining. There, it competes with harmful bacteria for space and nutrients. It produces organic acids that lower the pH of the gut, releases antimicrobial substances, and helps regulate the body's immune response.

All of these actions help in reviving the natural balance of bacteria in the gut, which often gets disrupted due to infection, antibiotic use, travel or stress.

Lactobacillus Rhamnosus is included in many over-the-counter probiotic products and as an adjunct in acute paediatric gastroenteritis, antibiotic-associated diarrhoea prevention, and infant colic or atopic dermatitis. However, it is not a substitute for prescribed therapy in severe infection.

Directions for Use

Take Lactobacillus Rhamnosus exactly as directed on the label or by your healthcare provider. Capsules, tablets, or sachets are usually taken once or twice daily, with or without food, swallowed with water. Sachet contents may be mixed with cool (not hot) water, milk, or juice, as heat inactivates the live bacteria. In the case of antibiotic-associated diarrhoea, take Lactobacillus Rhamnosus at least 2 hours apart from the antibiotic dose, and continue for at least 1 week after the antibiotic course is complete. For traveller's diarrhoea, start 2–3 days before departure and continue daily throughout the trip.

How it works

The gut is a house to trillions of microorganisms that influence digestion, mucosal barrier function, and systemic immunity. This balance of microbes often gets disrupted (dysbiosis), due to infection, antibiotics, stress, diet, or travel, resulting in diarrhoea, abdominal discomfort, and higher risk of infection. Lactobacillus Rhamnosus acts through several complementary mechanisms to restore balance and support host defences.

Lactobacillus Rhamnosus binds to intestinal epithelial cells, occupying attachment sites that pathogens such as enteropathogenic E. coli, Salmonella, and Clostridioides difficile would otherwise use and cause diarrhoea.

Second, it produces lactic and other organic acids, antimicrobial peptides, and hydrogen peroxide, which lower luminal pH and inhibit pathogens directly.

Third, it strengthens the epithelial barrier by stimulating epithelial cell survival and promoting tight-junction proteins, thereby reducing paracellular permeability.

Fourth, it modulates the gut's immune response by encouraging anti-inflammatory signals, promoting secretory IgA (the antibody that protects mucosal surfaces), and maintaining a balance between pro- and anti-inflammatory activity. All these actions help reduce the duration and severity of diarrhoea.

Drug-Food Interaction

Seek urgent medical attention if you experience consistent fever with chills, hypotension (low blood pressure), or unexplained severe illness, as these can be signs of systemic infection. Consult a doctor immediately if these signs appear in immunocompromised patients or those with central venous access.

What If You Forget to take Lactobacillus Rhamnosus?

If you miss a dose, take it as soon as you remember. If it is close to the next scheduled dose, skip the missed dose and continue normally. Do not double-dose. Consistent daily dosing is important for maintaining microbial colonies and clinical benefit, especially alongside antibiotics or for traveller's diarrhoea prevention.
