Ivermectin (Ivermectin) is a broad-spectrum antiparasitic medication used to treat internal parasitic infections as well as various inflammatory or parasitic skin conditions. It belongs to the avermectin class of medicines, which work by interfering with the nervous system and muscle function of specific parasites. Ivermectin is highly effective against several types of roundworms and external parasites like lice and mites. It is primarily available as oral tablets for systemic infections, while topical formulations, such as creams and lotions, are utilised for localised skin infestations like rosacea, scabies, and head lice.

Ivermectin is an anthelmintic and antiparasitic agent derived from the avermectin family. It is used to manage a variety of parasitic diseases by targeting the glutamate-gated chloride channels found in the nerve and muscle cells of invertebrates.

When administered orally, Ivermectin binds to these specific channels, increasing the permeability of the parasite's cell membrane to chloride ions. This leads to hyperpolarisation of the nerve or muscle cell, resulting in the paralysis and eventual death of the parasite. In conditions like onchocerciasis (river blindness), the medication effectively kills the immature worms (microfilariae) that cause skin and eye disease, although it does not typically kill the adult worms. For intestinal infections like strongyloidiasis, a single oral dose is often sufficient to clear the parasites from the digestive tract.

In topical applications, Ivermectin exerts both antiparasitic and anti-inflammatory effects. When used as a cream for rosacea, it helps reduce the inflammatory lesions (bumps and pimples) by potentially targeting Demodex mites that live on the skin and by suppressing certain inflammatory pathways. As a lotion for head lice, it paralyse the lice, making them unable to feed or survive.

Ivermectin is designed to be highly selective for parasites, as the specific chloride channels it targets are generally not present in the central nervous system of humans. Furthermore, the medication does not easily cross the human blood-brain barrier under normal conditions. This selectivity helps ensure that the medication effectively clears the infection while maintaining a favourable safety profile for the patient.
