Fluconazole is an oral medicine used to treat Type 2 Diabetes Mellitus (T2DM) in adults. It is prescribed when diet, exercise, or a single medicine are not enough to control blood sugar. The medicine combines Metformin and Vildagliptin. Metformin lowers blood sugar by reducing glucose production in the liver and improving the body’s response to insulin. Vildagliptin works by increasing incretin hormones, which help the pancreas release insulin and reduce glucagon after meals. These effects happen in a glucose-dependent manner, which lowers the risk of low blood sugar (hypoglycemia). Together, the combination helps maintain stable blood sugar throughout the day and supports long-term diabetes management.

Fluconazole is an oral anti-diabetic medication used in the management of Type 2 Diabetes Mellitus (T2DM) in adults whose blood sugar levels are not adequately controlled with lifestyle modifications alone or with a single anti-diabetic medicine. It combines Metformin, a biguanide, with Vildagliptin, a DPP-4 inhibitor (Dipeptidyl Peptidase-4 inhibitor), to provide effective and complementary blood glucose control.

Metformin acts primarily by reducing the amount of glucose produced by the liver and improving the body’s sensitivity to insulin, allowing glucose to be used more efficiently by muscle and fat tissues. It also slightly reduces the absorption of glucose from the intestine, helping to control post-meal blood sugar rises. Vildagliptin works by inhibiting the DPP-4 enzyme, which increases the levels of naturally occurring incretin hormones such as Glucagon-like peptide-1 (GLP-1) and Glucose-dependent insulinotropic polypeptide (GIP). These hormones help regulate blood sugar by enhancing insulin secretion and suppressing glucagon release following meals.

The actions of both medicines are complementary. Metformin addresses insulin resistance and excessive hepatic glucose production, while Vildagliptin enhances the body’s physiological insulin response in a glucose-dependent manner, reducing the likelihood of hypoglycemia when used without other glucose-lowering agents. This combination supports stable blood sugar control throughout the day and contributes to meaningful reductions in long-term markers of glycaemic control, such as glycated haemoglobin (HbA1c).

Fluconazole is generally well tolerated when taken as prescribed and is usually taken with or after meals to minimise gastrointestinal side effects commonly associated with Metformin. Regular monitoring of blood sugar levels and kidney function is recommended during treatment. By maintaining blood glucose within target ranges, Fluconazole supports long-term diabetes management and helps reduce the risk of complications affecting the nerves, kidneys, eyes, and cardiovascular system.

For patients who are not sufficiently controlled on Metformin or Vildagliptin alone, or those already taking both drugs as separate tablets.

Common Side Effects (mostly due to Metformin):

• Gastrointestinal issues: Nausea, vomiting, diarrhoea, abdominal pain, loss of appetite, or a metallic taste in the mouth. (These are best managed by taking the medicine with food.)
• Dizziness or Headache
• Tremor

Serious Side Effects (Stop taking the medicine and seek urgent medical attention if you experience):

• Lactic Acidosis (Metformin): A very rare but life-threatening condition. Symptoms include extreme fatigue, severe muscle pain, difficulty breathing, unusual sleepiness, or a slow heartbeat. Seek emergency help immediately.
• Pancreatitis (Vildagliptin): Severe and persistent stomach pain that may radiate to your back.
• Liver Dysfunction: Symptoms such as yellowing of the skin/eyes (jaundice), dark urine, or persistent nausea. (Vildagliptin requires periodic liver function monitoring.)
• Angioedema: Swelling of the face, lips, tongue, or throat.

• Dosage: Usually taken twice daily (one in the morning and one in the evening).
• Administration: The tablet MUST be taken with or just after a meal to minimise the stomach-related side effects associated with Metformin.
• Consistency: Swallow the tablet whole with a glass of water at the same time every day to maintain a steady effect.

Its effects are glucose-dependent (meaning insulin release is stimulated mainly when blood sugar is high, reducing the risk of hypoglycaemia).
• Together, Metformin and Vildagliptin provide complementary blood sugar control. Metformin addresses excessive glucose production and insulin resistance, while Vildagliptin enhances the natural insulin response after meals.

• Routine Labs: Your doctor will likely order blood tests for your liver and kidneys every few months. Don't skip these.
• B12 Monitoring: Long-term Metformin use can sometimes lower Vitamin B12 levels; ask your doctor about periodic checks or supplements.
• Sick Day Rules: If you have a severe fever, infection, or can't keep fluids down, contact your doctor; you may need to stop the medicine temporarily.
• Foot Care: Medication is only one part of diabetes care. Inspect your feet daily and maintain regular dental and eye exams.

Store in a cool, dry place below 30°C. Protect from moisture and light by keeping tablets in their original blister pack. Keep away from children.

MANDATORY INTERACTION.

Always take with meals. High-fibre foods may slightly slow Metformin absorption, but are generally encouraged as part of a diabetic diet.

• Iodinated Contrast Media: Stop taking Fluconazole 48 hours before and after any X-ray or CT scan involving contrast dye to protect kidney function.
• Corticosteroids & Diuretics: These can raise blood sugar levels, potentially requiring a dose adjustment of your diabetes medication.
• Sulfonylureas: Taking Fluconazole with drugs like Glimepiride increases the risk of hypoglycaemia.
• ACE Inhibitors: May slightly increase the risk of angioedema (swelling) when taken with Vildagliptin.

• Severe Renal Impairment: Risk of drug accumulation and lactic acidosis.
• Hepatic Impairment: Risk of liver toxicity with Vildagliptin; Metformin risk in liver failure.
• Dehydration: Severe vomiting or diarrhoea can lead to kidney issues; stop the medicine and consult a doctor.
• Heart Failure: Use with caution in patients with stable heart failure; avoid in acute or unstable cases.

Standard dosing is one tablet twice daily. Do not exceed the dose prescribed by your healthcare provider.

An overdose can lead to lactic acidosis or severe gastrointestinal distress. Action Required: Seek emergency medical care or contact a poison control centre immediately if an overdose is suspected.

Take the missed dose with food as soon as you remember. If it is nearly time for the next dose, skip the missed one. Never "double up" on doses.
