Acarbose is an oral antidiabetic medication containing Acarbose. It is specifically designed to manage blood glucose levels in adults with type 2 diabetes mellitus. Unlike other diabetes medications that act on the pancreas or liver, Acarbose works directly in the digestive tract to slow down the breakdown of carbohydrates from food into sugar. This results in a smaller rise in blood glucose levels immediately after meals. It is often used as an addition to diet and exercise, either alone or in combination with other glucose-lowering agents like metformin or insulin.

Acarbose belongs to a class of drugs known as alpha-glucosidase inhibitors. It is a unique therapeutic tool for controlling spikes in blood sugar that occur after eating.

Composition and Mechanism: Each tablet contains 50mg of Acarbose. The medication acts as a competitive, reversible inhibitor of the pancreatic alpha-amylase and membrane-bound intestinal alpha-glucoside hydrolase enzymes. • Limit Junk Carbs: The more refined sugar you eat, the more gas you will experience. Stick to complex grains.

• Patience: Stomach side effects usually improve significantly after the first 2–4 weeks of use.

Store Acarbose at room temperature (below 25°C). Keep the tablets in their original moisture-proof blister pack, as Acarbose can absorb moisture from the air and become discoloured. Keep out of reach of children.

• Table Sugar (Sucrose): Causes severe gas and liquid stools because it cannot be absorbed properly.

• Digestive Enzymes: Avoid taking charcoal or digestive enzyme supplements at the same time, as they can stop Acarbose from working.

• Chronic Kidney Disease: Not recommended for patients with severe renal impairment.

• Cirrhosis: Use with caution; monitor liver enzymes closely.

The dosage is highly individualised and determined by a physician. Most patients begin with an Initial Dose of 50mg once daily. This low start allows the digestive system to adapt to the changes in carbohydrate fermentation.

Over several weeks, the doctor will transition you to a Maintenance Dose, typically 50mg to 100mg three times daily (with each main meal). The Maximum Dose rarely exceeds 300mg per day. Regular HbA1c tests ensure you stay within a safe therapeutic range; never change your dose without medical advice.

If you remember during the meal, take the dose immediately. If the meal is finished, skip the dose. Do not take a 50mg tablet between meals or double the dose later, as it will not work without food and will only cause severe stomach upset.
