Quick Summary

Vasograin Ec Tablet 20s is a four-component combination for the acute treatment of migraine and tension-type headaches. Ergotamine constricts widened (dilated) blood vessels inside your head (cranial blood vessels) to relieve throbbing pain, while caffeine enhances ergotamine's absorption, paracetamol provides analgesic relief, and prochlorperazine controls nausea and vomiting. Together, they tackle the pain, vascular, and gastrointestinal aspects of headache episodes in a single formulation.

Detailed Description

This fixed-dose combination in Vasograin Ec Tablet 20s targets multiple aspects of headache pathophysiology all at once. Ergotamine acts as a non-selective agonist at serotonin, dopamine, and alpha-1 adrenergic receptors, narrowing dilated cranial blood vessels and providing relief. Caffeine amplifies ergotamine's vasoconstrictive effect and increases its oral absorption. Paracetamol inhibits prostaglandin synthesis, managing pain, while Prochlorperazine blocks dopamine D2 receptors in the brainstem to prevent nausea, vomiting, and sensory sensitivity during attacks.

This medicine is for short-term use and must be used when needed. It is most effective at the earliest onset of an attack. Overuse of Vasograin Ec Tablet 20s more than 2–3 days per week causes medication overuse headache (MOH). This medicine must not be taken during pregnancy, or by people suffering from cardiovascular disease, peripheral vascular disease, uncontrolled hypertension, or significant liver or kidney impairment.

Uses of Vasograin Ec Tablet 20s

Acute migraine with or without aura: To stop vascular headache at the earliest onset

Tension-type headache

For moderate-to-severe tension headache, particularly when accompanied by nausea

Vascular headache and cluster headache

To manage headaches due to constriction of blood vessels in the cranium (head).

Migraine-associated nausea and vomiting

To manage gastrointestinal symptoms associated with migraine and to improve comfort and absorption

Benefits of Vasograin Ec Tablet 20s

  • Multi-component relief in a single formulation addressing vascular pain, general pain, and nausea together
  • Ergotamine targets cranial vasodilation (which is often called the driver of throbbing pain), a mechanism that general analgesics cannot replicate
  • Caffeine enhances ergotamine's absorption and effect without requiring a higher ergotamine dose
  • Paracetamol covers residual headache pain, facial pressure, and body discomfort
  • Prochlorperazine controls nausea and vomiting, improving medicine retention and absorption during attacks
  • Most effective when taken at the earliest onset, with potential to abort an attack before peak severity

Directions for Use

Vasograin Ec Tablet 20s is available as oral tablets. You can swallow it with a full glass of water, with or without food; however, a light meal may reduce stomach upset. Take at the first sign of a headache as it is most effective before peak intensity.

It must be taken as per your doctor’s direction. Do not exceed the prescribed maximum dose per attack or per week. Do not use more than 2 to 3 days per week to avoid MOH. Do not take Vasograin Ec Tablet 20s within 24 hours of any triptan. Do not combine it with other paracetamol-containing products.

How it Works

Ergotamine acts on neurotransmitters (serotonin and dopamine) and alpha-1 receptors to relax widened blood vessels in the head and reduce migraine pain signals.

Caffeine increases ergotamine's effect of blood vessel constriction (vasoconstrictive effect) and also increases oral absorption by inhibiting first-pass metabolism. It also provides mild CNS stimulation that partially counteracts prochlorperazine's sedation.

When your body gets injured or infected, the cell membranes release a fatty acid called arachidonic acid. The COX enzyme converts that into prostaglandins. Prostaglandins are the chemicals that are responsible for pain, fever, swelling, and inflammation at the site. Paracetamol inhibits the cyclooxygenase (COX) pathway, reducing the production of prostaglandins, thereby decreasing pain perception.

Prochlorperazine blocks dopamine D2 receptors in the chemoreceptor trigger zone present in the brainstem (in medulla oblongata), preventing nausea and vomiting. It also provides sedation that reduces agitation and sensory sensitivity during attacks by blocking the Histamine H1-receptors.

Safety Advice for Vasograin Ec Tablet 20s

Pregnancy

Caution

Ergotamine can stimulate uterine contractions and hamper placental blood flow, causing fetal harm.

Quick Tips for Vasograin Ec Tablet 20s

  • Take Vasograin Ec Tablet 20s at the very first sign of a headache attack, as it works best when taken early
  • Never exceed the prescribed dose per attack or per week, as overuse causes MOH and raises the risk of ergotism
  • Do not take within 24 hours of any triptan (sumatriptan, rizatriptan, zolmitriptan) as it increases the risk of vasospasm (sudden spasm of blood vessels)
  • Never take another paracetamol-containing product simultaneously. Check the label of all cold, flu, and pain medicines before intake
  • Avoid smoking during treatment, as nicotine significantly increases the risk of peripheral ischaemia when used while taking ergotamine
  • Limit additional caffeine from food and drink, as Vasograin Ec Tablet 20s already contains caffeine
  • If fingers or toes become cold, numb, or painful, stop Vasograin Ec Tablet 20s immediately and contact your doctor
  • Do not use Vasograin Ec Tablet 20s if you’ve been diagnosed with cardiovascular disease, peripheral vascular disease, uncontrolled hypertension, or significant liver/kidney disease without medical clearance

Storage Advice

Store at room temperature between 20-25°C in a dry place away from direct sunlight and moisture. Do not freeze. Keep out of reach of children. Do not use beyond the expiry date.

Drug-Food Interaction

  • Alcohol (Severe): Alcohol consumption raises the risk of paracetamol-induced liver toxicity and also heightens the likelihood of prochlorperazine's sedative and hypotensive effects. Hence, it must be avoided.
  • Grapefruit and grapefruit juice (Caution): Inhibits CYP3A4, raising ergotamine levels and risk of vasospasm and ergotoxicity. Hence, it must be avoided.
  • Caffeine-containing foods and drinks (Moderate): Coffee, tea, energy drinks, and chocolate add to the caffeine content already present in this medicine, risking palpitations, tremors, and insomnia.

Interactions with Other Drugs

Triptans

Severe

Triptans (like sumatriptan, rizatriptan, zolmitriptan) can cause additional vasospasm and increase the risk of cerebral or peripheral ischaemia

Strong CYP3A4 inhibitors

Severe

Medicines like erythromycin, ketoconazole & ritonavir block the metabolism of ergotamine, leading to fatal accumulation, vasospasm and ergotism.

Other paracetamol-containing medicines

Severe

Increases total paracetamol dose and risk of liver toxicity

MAO inhibitors (phenelzine, tranylcypromine, linezolid)

Severe

Risk of severe hypertensive crisis

Beta-blockers

Caution

Increased risk of ergotamine's vasoconstrictive action and peripheral ischaemia

CNS depressants (sedatives, opioids, benzodiazepines)

Caution

Increased risk of prochlorperazine's sedative, falls and respiratory depression

Sympathomimetics

Caution

Medicines like pseudoephedrine or ephedrine cause additional vasoconstriction, raising the risk of severe hypertension or peripheral ischaemia

Warfarin and anticoagulants

Caution

Paracetamol may enhance the anticoagulant effect

Anticonvulsants (carbamazepine, phenytoin)

Caution

Lowers ergotamine levels via CYP3A4 induction.

Increased risk of frequent seizure episodes.

Drug-Disease Interactions

  • Cardiovascular disease, coronary artery disease, angina: Ergotamine can result in coronary vasospasm.
  • Peripheral vascular disease, Raynaud's, Buerger's: Ergotamine worsens arterial insufficiency (low blood flow to the arteries) and increases the risk of gangrene.
  • Uncontrolled hypertension: Ergotamine raises blood pressure.
  • Liver disease or chronic alcohol use: Ergotamine accumulation may occur in those with impaired liver. Also, the paracetamol hepatotoxicity risk is high in such people.
  • Severe renal impairment: Impaired ergotamine elimination in people with impaired kidneys increases ergotism risk.
  • Epilepsy or seizure disorders: Prochlorperazine lowers seizure threshold, potentially increasing the risk of more episodes.
  • Parkinson's disease: Prochlorperazine worsens symptoms of Parkinson’s by blocking dopamine receptors.
  • Narrow-angle glaucoma: Anticholinergic effects may raise intraocular pressure. This effect of Prochlorperazine inhibits muscarinic receptors in the iris, leading to pupil dilation, relaxation of ciliary muscles, thickening of the iris, blockage of drain, and finally impaired outflow of vitreous humour, thereby raising intraocular pressure.
  • Diabetes mellitus: Caffeine may affect glucose regulation.

Overdose

  • Paracetamol overdose is the most dangerous risk and can be fatal even if the person feels completely alright in the beginning. Early signs include nausea, vomiting, loss of appetite, and abdominal pain. After 24 to 72 hours, the person may experience delayed symptoms such as right upper abdominal pain, dark urine, and jaundice, indicating liver damage.
  • Ergotamine overdose can result in ergotism, which is a serious condition and if left untreated, can progress to gangrene.
  • Prochlorperazine overdose may cause extreme sedation, involuntary muscle movements or rigidity, seizures, arrhythmias (irregular heart rate), and hypotension (low blood pressure). Stop the medicine immediately and consult a medical professional.

What If You Forget to take Vasograin Ec Tablet 20s?

Since this medicine is taken as needed at headache onset, a missed dose typically does not apply. If you have been recommended a scheduled regimen, take the missed dose as soon as you remember, unless the next dose is due within 2 hours. In such cases, it’s better to skip it than take double the dose.
